Addendum
This document serves as an Addendum to the Statement of Work provided to the City of Fort Collins
on August 14, 2013. That Statement of Work covers the development of two Web -based training
courses on the topic of Title VI of the Civil Rights Act.
Scone
Through its hosting partner eApps, Monarch Media will provide one year of Web hosting services
for the two courses. The Web hosting set up will be a Linux-based virtual machine in a cloud
hosting environment. The technical specifications for the hosting plan will include:
• 2048 MB of RAM
• 3 CPU cores
• 15 GB of primary disk space
• 175 GB data transfer
• 1 IP address
• CentOS 6 64 bit LAMP operating system template
The hosting services will also include 24/7/365 server support from eApps available by phone,
email, or chat.
Monarch Media will be responsible for setting up and configuring the hosting plan and uploading
the course files to it. The hosting plan will be purchased within two business days of Monarch
Media receiving a countersigned copy of this Addendum and an updated purchase order number.
Project Cost
The cost for one year (12 months) of hosting with the configuration noted above will be
$948.00. This amount will be billed to Fort Collins as part of Monarch Media's monthly
invoice for the month during which the hosting plan is purchased.
Terms & Conditions
If the amount of content or number of users for the hosted courses exceeds initial estimates,
the City of Fort Collins may need to purchase additional RAM, disk space, data transfer, or
CPU shares to ensure optimal performance for the hosted courses.
Additional hosting or support requirements not covered in this Addendum may require a
separate addendum or agreement to implement.
Fort Collins will be responsible for alerting Monarch Media of any changes in the
requirements for these hosting services.
Please note that this Addendum and the original Statement of Work can be updated or changed only
if agreed upon in writing by both Monarch Media, Inc., and by the City of Fort Collins. Thank you for
the opportunity to provide Fort Collins with this Addendum.
