Request for Scope Modification to Work Order NAD-2013-002
Dear Rick:
This letter outlines the additional 2013 scope items that are associated with the design -build projects
and outside of the Construction phase. As we have discussed, we would appreciate if the current
Professional Services Agreement could be modified to cover these costs.
McMurry Phase 2 -- $13,465.00
• City Floodplain Use Permit Preparation and Submittal
• City Erosion Permit Preparation and Submittal
• Colorado Construction SW Permit Prep and Submittal
• Finalize Revised Toe Protection
• Flood -related modification
• Coordinate City Riprap work
• Finalize Design, Field Visit
• Final Plan Set
• Evaluate results of combined hydraulic modeling
Sterling -- $ 6,400.00
• Colorado Construction SW Permit Prep and Subrr
• County Floodplain Board Condition Response
• ELCO and CWT Continued Coordination
• Finalize Design, Field Visit
• Grading revisions Final Plan Set
Total Add -On Scope = $19,865.00
Please note that most of the Sterling items were completed in September in order to keep the project moving
forward and with the understanding that a cost modification would be forthcoming. Also note, that the McMurry
project tasks. Include assessment and plan revisions to address the impact of the mid -September flood damage.
