HomeMy WebLinkAboutCHANGE ORDER - AGREEMENT MISC - DITESCO LLCCHANGE ORDER NO. 1 PROJECT TITLE: Poudre River Public Library District - Library Park Masterplan CONTRACTOR: Ditesco, LLC PROJECT NUMBER: n/a PURCHASE ORDER NO.: q DESCRIPTION: (See below and attached) Reason for change: The reason for the change to this contract is to allow continuation of project and construction management services for the Library Park Masterplan. Ditesco was initially contracted to provide these services for the first phase of the Library Park Masterplan including the entry Plaza, Story Tree, Checkerboard and Arboretum. This is scheduled for completion in spring 2013. The second phase of f the Masterplan is now funded and is planned to be fully complete by December 2013. The original scope j of work and agreement is attached hereto as Attachment B. 2. Description of Change: The description of the contract change is attached hereto as Attachment A. The contract change includes full project management services including oversight, coordination, plan and specification review, bidding assistance, construction observation and management, budget, schedule and work management. Work authorized through this change order shall be complete according to the following schedule: i Design and Procurement Management - complete by May 10, 2013 Construction Mangement complete by Fall 2013. 3. Change in Contract Cost: $43,555.00 4.
