DATED: June 15, 2013 Work Order No. 10 Purchase Order No. Project title: Bridge Inspection, Maintenance, Repair and Replacement Program Arthur Ditch Manhole Retrofits at Lake Street and Prospect Road Project Commencement Date: June 6, 2013 Completion Date: November 30, 2013 Maximum Fee: $26,906.97 PROJECT DESCRIPTION The purpose of this project is to provide final design services, including constriction documents, for the retrofit of the Arthur Ditch Concrete Box Culverts (CBCs) at Lake Street and Prospect Road Fort Collins, Colorado. The retrofits will provide new manholes in the top slab of these structures for the purposes of improving personnel, access and safety. The existing culvert access to the south of Prospect Road is a two -foot diameter manhole with a tapered transition to an offset four -foot diameter bottom riser section. The existing manhole is located in the top slab on the western side of the CBC. This manhole is to be replaced with a three-foot diameter manhole access in the same.area as the existing access, although located on tine eastern side of the culvert. Currently no culvert access is provided to the north of Prospect Road. A three-foot diameter manhole access is to. be provided at this location as part of this project. This proposed access is in parking lot adjacent to a CSU parking garage, a sidewalk, and nearby businesses. The CITY will direct BAKER with the exact location of this new manhole. The manhole cover will be rated for traffic loading. At Lake Street, culvert access is already currently provided by a manhole at approximately 50 feet south of the roadway. An additional access point is to be provided to the north of the roadway, adjacent to CSU facilities and public walkways through existing embankment fill. All three manhole retrofits will be similar in nature. A portion of the top slab of the culvert will be removed and replaced with new manhole accesses. This scope of work defines the effort required to provide bid documents for the retrofit of the top slab of the culvert to include manhole access at these locations. No other rehabilitation work is anticipated at this time. Exhibit A -Work Order Form —Work Order No.10 Bridge Inspection, Maintenance, Repair and Replacement Program —Arthur Ditch Manhole Retrofit Project SCOPE OF SERVICES: The scope of services for this project is to perform design services and prepare construction documents for the retrofit of the Arthur Ditch Concrete Box Culvert access locations adjacent to Lake Sheet and Prospect Road. This work includes the replacement of portions of the top slab sections as necessary to accommodate new manholes. BAKER will provide design services including: project management, Feld inspection, structural design, cost estimating, and bidding/construction assistance. All work will be based upon information determined from the original construction plans, inspection reports, and field inspections. Services not included in this scope of work include and provided by the CITY, if necessary, are: traffic control including pedestrian facilities, erosion control, utility identification and coordination, topographical survey, envirormrental coordination and permitting, and right-of-way services. Design criteria, standard details, and specifications shall first follow CDOT guidelines and secondly follow CITY guidelines. Final Construction Documents shall be consistent with CDOT requirements. The design process will only include one review by the CITY. The final product for this scope is a complete construction bid package (plans, specification, and estimate) for modifications to the Arthur Ditch Concrete Box Culverts adjacent to Lake Street and Prospect Road. I. PROJECT ADMINISTRATION Upon notice to proceed, BAKER will perform project administration duties throughout the length of the contract. A. Invoices will be provided to the CITY on a monthly basis and will include a billing summary and a progress report for work performed during the billing period. B. All documents will receive a quality assurance and quality control (QA/QC) review, in accordance with the BAKER Way Manual, prior to submittal. C. The following meetings are anticipated: rr Weekly Status Meetings by Phone rr One Pre -Design Meeting (Fort Collins) n One Design Review Meeting (Fort Collins) TT One Pre -Construction Meeting (Fort Collins) D. BAKER will prepare documentation of meetings and telephone conversations and provide them to the CITY for review and recording keeping. II. FINAL DESIGN The final design phase includes preparation construction document preparation for the project. Work elements include: A. Structural Design — BAKER shall prepare engineering design calculations as necessary. Independent design calculations shall be prepared in accordance with CDOT guidelines and procedures. Design calculations shall be submitted to the CITY upon request. A load rating package and construction field package will not be included in this scope. Construction Phasing Plan — BAKER will prepare constriction documents assuming no construction phasing considerations are necessary. C. Traffic Control Plan — The CITY is responsible for determination of traffic control approach and all devices, and implementation during construction. BAKER will prepare construction documents assuming no impacts to adjacent roadways and limited impacts to adjacent sidewalks and parking lots by use of shoring. D. Erosion Control Plan — At direction from the CITY, an erosion control plan will not be prepared as part of the construction documents. E. Construction Plans — BAKER shall prepare construction plans for the full scope of the project. This work order includes all work necessary to develop final construction plans. The plans shall be QC. reviewed prior to submittal to the CITY. Constriction plans shall be prepared in the Microstation format. Project Specifications — Project specifications will not be provided separately but included in the plans. All work shall be in accordance with the 2011 Colorado Department of Transportation (CDOT) Standard Specifications for Road and Bridge Construction as amended in the construction documents. The construction specifications shall follow CDOT specifications where applicable and CITY specifications, otherwise. G. Construction Cost Estimate — BAKER will prepare quantities and an engineer's estimate of probable construction costs for all work items anticipated on the project. III. ADVERTISEMENT The advertisement design phase includes revisions to the final design and construction documents in response to comments provided by the CITY from the design review. Work elements include: A. Structural Design — BAKER shall revise the engineering design according to comments from the CITY. All documents shall be signed and sealed. B. Contract Documents — BAKER shall revise constriction plans and project specifications according to comments from the CITY. C. Construction Cost Estimate — BAKER will prepare quantities and an engineer's estimate of probable construction costs for all work items anticipated on the project. Assistance with Project Bidding - It is anticipated that a traditional bidding process will be followed for this project. BAKER will attend a pre -construction meeting with the CITY and Contractor. E. Preparation of a Complete Plan Set for Review by the CITY — Prepare a 100% plan set for review by CITY staff. The plans shall be prepared as a stand-alone construction document. Any information that would typically be conveyed through project special Provisions will be included as notes as part of the plan set. Only one CITY review will be provided. F. Preparation of "For Construction" Plan Set — Prepare a final plan set package that incorporates any revisions from CITY the review. Plans shall be marked "FOR CONSTRUCTION, (DATE)". All documents that are signed and sealed. G. Construction Administration Assistance — BAKER will provide construction administration and design support services. Construction administration effort may include: RFI responses, as -built plans, shop drawing review, and design support, amongst others. For estimating purposes, it is assumed that this work.will require no more than 20 hours of labor from BAKER. IV. REFERENCES AND DESIGN CRITERIA The following references and design criteria shall be used throughout all phases of the project: A. All structural design shall follow the load and resistance factor design (LRFD) methodology. Design shall be in accordance with the American Association of State Highway and Transportation Officials (AASHTO) LRFD Bridge Design Specifications, 6th Edition (2012) with interims througli 2013. B. Construction document plans shall be prepared in accordance with the latest version of the CDOT Bridge Detailing Manual. C. Construction specifications will be prepared in accordance with the 2011 Colorado Department of Transportation (CDOT) Standard Specifications for Road and Bridge Construction. V. DELIVERABLES The following is an estimate of the deliverables required for this project. As the project develops, the type and number of deliverables may vary. Refer to the detailed discussions above for further detail. All copies shall be submitted in hard copy and .pdf format; plans are 11x17, all others 8% xl 1, except as noted otherwise.
