This work includes design, procurement, installation and construction of the NRRC (aka Spring Creek) Overpass as is shown in the attached Mason Grade Separation at NRRC. Change Order 4-12-13 plans and Mason Grade Separation at NRRC-Change Order No 5 specification special provisions. Elements of construction not specifically outlined in the Mason Grade Separation at NRRC-Change Order No 5 specifications and special provisions shall be specified, constructed and warranted according to the Specifications and Contract Documents for the MAX Project — Bid #7332 as amended herein. Specifically, the work consists of excavation, embankment, caisson installation, reinforcing steel, structural concrete, sitework, railing, traffic control, wall construction, lighting, site furnishings, mobilization and general conditions. Through Work Change Directive (WCD) 051, MAX Proiect — Bid #7332 contract specifications and construction plans have been formally amended to include the following additions. Snecifications: Revision of Section 504 Concrete Block Facing MSE Wall Revision of Section 514 Pedestrian Railing (Steel) Revision of Section 601 Structural Concrete Revision of Section 613 Lighting and Power Equipment Revision of Section 715 Lighting Materials Revision of Section 622 Pedestrian Ramp Structure Revision of Section 622 Rest Area and Building (Site Furnishings) Revision of Section 630 Construction Traffic Control Construction Plans: Contract Drawing Sheets 1-46 Change in Contract Value: The contract value is being increased by $1,779,894.43. Total Contract Chan a -this cha WCD051 — Mason Grade Separation $1,779,894.43 Total Change Order: $1,779,894.43 Page 2 of 4 Change in Contract Time: The MAX Project Bid #7332 contract documents; Section 00520 — Agreement, Article 3; Part 3.2 Liquidated Damages is hereby modified as follows. 1) Substantial Completion: Add after the first paragraph: Eight Hundred Dollars ($800) for each calendar day or fraction thereof that expires after July 31, 2013 for Substantial Completion Milestone 1 of the East and West Plaza Piers as shown on the Mason Grade Separation at NRRC. Change Order4-12-13; Sheets 16 through 32. Substantial Completion Milestone 1 shall be defined as completion of the East and West Plaza Piers, ready for inspection, concrete initially cured, forms stripped and available for conformational measurements of bearing for steel bridge fabrication. Two Thousand Dollars ($2,000) for each calendar day or fraction thereof that expires after October 31, 2013 for Substantial Completion Milestone 2 of the Mason Grade Separation as shown on the Mason Grade Separation at NRRC, Change Order 4-12-13 plans and as defined in the Mason Grade Separation at NRRC-Change Order No 5 specifications and the MAX Project — Bid #7332 contract documents. Substantial Completion Milestone 2 shall be defined as completion of the Mason Grade Separation project as defined in the plans and specifications referenced above. This portion of the contract (WCD051/Change Order No. 5) shall be ready for final acceptance. 2) Final Acceptance: Add after first paragraph: After Substantial Completion of Milestone 2, Five Hundred Dollars ($500) for each calendar day or fraction thereof that expires after a 15 calendar day period for Final Payment and Acceptance. Final Acceptance of the Mason Grade Separation shall be defined as all work complete, including soil preparation, seeding and construction punch list(s) ready for final acceptance and beneficial use of the site and facility.
