Exhibit "A" WORK ORDER FORM PURSUANT TO AN AGREEMENT BETWEEN THE CITY OF FORT COLLINS AND MICHAEL BAKER JR., INC. DATED: April 2, 2013 PROJECT DESCRIPTION This scope of work is for a number of miscellaneous inspection tasks, including: • An emergency bridge inspection and related services of structure number MLD-WHTM-A • Additional measurements and geometric investigation of structure numbers PRST-W-CTRE and LAKE-W-CNTR • New bridge inspection and related services of structure number OLIVE-LOOMIS • Locate, expose, and mark buried manholes on Arthur Ditch and relay location to the City (CHRY- GRNT, CHRY-SYCM-A, MAP-WHTM-A, MTN-W-WHTM, MGNL-OLIV-A/ OLIVE-LOOMIS (same access)) This work order is performed under the master agreement for the City's Bridge Inspection, Maintenance, Repair, and Replacement Program. SCOPE OF SERVICES: The scope of work for each individual work elements is described below. Emergency Inspection of MLD-WHTM-A It was discovered by the City that a sink hole was formed next to the existing Arthur Ditch structure, causing a local failure of the adjacent sidewalk. The source of the sink hole is unknown and located within the City's Right -of- Way. Since Arthur Ditch is scheduled to run water within the second week of April, it is an urgent matter to find the cause of the sink hole. Baker is tasked to provide the City of Fort Collins an Emergency Bridge Inspection with the intent to investigate potential causes of the sink hole. Work elements include: Project administration, field inspection, coordination with the City regarding immediate findings and to institute immediate repairs, photos and measurements, reporting, documentation of the process through a memorandum, and coordination with the City regarding the recommended course of action. A load rating will not be required. Additional Measurements of PRST-W-CTRE and LAKE-W-CNTR The City is considering a future retrofit of these two existing Arthur Ditch structures to improve the access points. New manholes will be provided at the north end of both structures, while the south manhole will be upsized on the PRST-W-CTRE structure. In preparation for this work, the City has requested that Baker provide more detailed field measurements of these structures for use with that future design project. Work elements include: Project administration, field inspection, photos and measurements, and documentation of the measurements in cad details. Exhibit A - Work Order Form — Work Order No. 8 Bridge Inspection, Maintenance, Repair and Replacement Program —Emergency Inspection & Miscellaneous Inspection Elements Bridge Inspection of OLIVE-LOOMIS ' The City has requested that Baker provide an inspection of the existing Arthur Ditch structure. Although the structure is not new, this will be the first such inspection and thus categorized as "new inspection". The inspection will extend from the manhole in the Alley to Olive Street, approximately 250' in length. Work elements include: Project administration, field inspection, photos and measurements, and documentation of the process through a memorandum. The memorandum will be included with the other structures of Work Orders 6 & 7. Locate Buried Manholes for CHRY-GRNT CHRY-SYCM-A MAP-WHTM-A, MTN-W-WHTM, MGNL-OLIV-A/ OLIVE-LOOMIS The City has requested that Balser locate, expose, and mark buried manholes on Arthur Ditch at various locations. Manhole location information will be presented to the City in a memorandum which will include sketches noting the approximate location of the manholes relative to the structure. Work elements include: field locating, exposing, and marking manhole locations, and documentation of the locations with a memorandum. The memorandum will be included with the other structures of Work Orders 6 & 7. GUIDELINES AND REFERENCES All inspections will be conducted in accordance with current NBIS standards and supervised by personnel meeting the requirements outlined in the Code of Federal Regulations, 23 CFR 650.307.
