The Reason and Description of the Contract Change Is as Follows:
The purpose of this change order Is to redefine the substantial completion and final completion definition and dates,
incorporate unit price and alternate items into the work , and establish parameters for final protect completion. The nev
substantial completion date Is to be December 31st, 2012. On this dale all of the contractor's equipment and tools will
be removed from the site with final punch list and unit price Items finished. The final acceptance date for final
reconciliation will be two weeks after, on January 13, 2013. A detailed change to the contract definition of subslantial
completion and final completion is attached. A total of (21) calendar days are added to the contract. 6) days for
inclement vreather, (5) days for difficulties encountered in waste stream separation and encountered ACM, and (10)
days for work area alteration for public use.

Date: December 27, 2012
Description of Change:
The purpose of this change order is to redefine substantial completion and final completion, incorporate unit price
and alternate Items Into the work, and establish parameters for final project completion.
Changes to Closeout Procedures are as follows:
DIVISION 01, Section 0177 00 CLOSEOUT PROCEDURES. Section 1.8, Paragraph A is hereby modified in its
entirety.
A. Substantial Completion shall not be allowed until each and every component of the project has
been completed and approved by Engineer. When Contractor considers the Work to be
substantially complete, he shall submit to Engineer:
i. A written notice that the Work or designated portion thereof is substantially complete
2. A list of items to be completed or corrected .
a. Substantial Completion shall be defined as the entire Elks Lodge building removed, all
damaged Infrastructure repaired, all work complete, demobilization complete, site
cleaned and ready for owner acceptance.
DIVISION 01 — GENERAL REQUIREMENTS 01 77 00 CLOSEOUT PROCEDURES Section 1.9 — FINAL
INSPECTION — Paragraph A is hereby modified in its entirety.
A. When Contractor considers the Work is complete, the Contractor shall submit written certification that:
1. Contract Documents have been reviewed
2. Work has been inspected for compliance with Contract Documents
3. Work has been completed in accordance with Contract Documents
4. Work is completed and ready for final Inspection
a. Final Acceptance shall be defined as all site work complete, all salvageable materials
delivered and inventoried; contract reconciled and final project closeout completed.
