Box 580 Fort Collins, Colorado 80522-0580 RE: Proposal for Vapor Mitigation System Support Northside Aztlan Community Center 112 Willow Street, Fort Collins, Colorado WALS11 Proposal Number: WA-000055-0048-OOTTO Dear Matt: I_e' /a507/4 /e_2� -* va6 fila Walsh Environmental Scientists and: Engineers, LLC (WALSH) is pleased to submit this proposal to provide vapor mitigation system (VMS) support activities at the Northside Aztlan Community Center located.at 112 Willow Street in Fort Collins, Colorado. The purpose of this project is to assess the current operating status of the VMS and recommend operations and maintenance modifications, if applicable. Our proposed scope of work is presented below: Scope of Work - VMS Support • Review VMS construction drawings and specifications. • Review recent VMS operations, maintenance and monitoring (OM&M) inspection data collected by City representatives. • Perform site visit and inspect components of VMS including interior flooring surfaces, gas detectors, sub -slab vacuum probes, control panel and system housing, as well as the exterior (roof mounted) blower, vent pipes, etc. Blower data such as flow rates, operating vacuums, and air temperature will be recorded. It is assumed that City technicians who are trained in system operating, inspection, and recordkeeping procedures will accompany WALSH personnel during the site visit. Furthermore, it is assumed that City representatives will provide documentation of any mechanical repairs/modifications to the VMS post installation. • Provide verbal recommendations on OM&M modifications, if applicable. • Prepare letter report summarizing VMS inspection activities. Letter report will include copies of field notes, checklists, tables, photographs, etc., as applicable. Submit electronic (Adobe PDF format) version of final letter report. • Perform one supplemental site visit/inspection and prepare letter report. The estimated costs to complete the scope of work outlined in the scope of work is $3,923. WALSH proposes to complete the tasks on a time and materials basis. Specific costs to complete the specified tasks are detailed on the attached spreadsheet. At certain times during the operation of the VMS, subcontractors may need to trouble shoot significant problems and/or make repairs to equipment. Examples of subcontractors that may be needed include vendors of the equipment, an electrician, or a blower repair technician. This proposal also assumes that the WALSH representative will not perform calibration of gas sensors and that the person performing the initial site visit/inspection will also performs tie supplemental site visit/inspection.
