Per the proposal requirements we have addressed all of the information requested for your use in the
evaluation and assessment of our qualifications.
a. Company history and experience in installation of lighting
• Rocky Mountain Repertory Theater, Grand Lake, CO

This project was a 12,000 sq. ft. new construction, 296-seat facility including lobby, backstage
area, dressing rooms, set storage, enhanced sound and advanced rigging systems and lighting
components.
• Fort Collins Discovery Museum, Fort Collins, CO

This is a new 45,000 sq. ft. building featuring over 11,000 sq. ft. of permanent exhibit space,
5,000 sq. ft. changing exhibit space, classrooms, a custom "Digital Dome" space, archive areas,
shops and administrative offices.
• Longmont High School Auditorium, Longmont, CO

This project was a remodel of the existing high school auditorium to include new house and
stage lighting consisting of an extensive lighting control system.
• CSU — Recreation Center, Fort Collins, CO

This project was a new recreation center and remodel to the existing building, including
decorative lighting and specialty lighting throughout to enhance the building's features.
• Midtown Arts, Fort Collins, CO

This project was a remodel of an existing building into a dinner theater with specialty lighting
installed. This project consists of the main stage, the 236-seat dinner theater, a 200+ seat
ballroom, rehearsal/studio/classroom, and a second 200 seal auditorium for outside
performances/meetings/lectures, a kitchen, grand hall, bar and lounge, and central box office.
b. Company representatives
Jerry Palaszewski; Project Manager — Will oversee the project from GMP to project
completion including change orders, operation and maintenance manuals, and as -built drawings.
He has held positions as project manager, field superintendent, safety manager and purchasing
manager. Jerry is experienced in purchasing, recruiting and training personnel, tracking job cost
to ensure job stays in budget, coordination of manpower to ensure timely project completion and
he has extended knowledge on various safety policies and procedures. His experience in new
construction as well as remodel/additions allows him to quickly and efficiently handle all

requests for changes to the projects, coordinate meetings as needed with field personnel, follow-
through with tracking of material to ensure that there is no disruption to the construction
schedule. Jem, holds active Master Electrician's licenses in Colorado and Wyoming and is an
OSHA Outreach Trainer.
Bevan Olesky; Project Foreman — Will oversee all field operations atjobsite for electrical
installations. He has worked as project supervisor on various projects of up to $1 million.
Bevan recently finished up the Super Vac project. A few of his projects as foreman include:
Midtown Arts, Vestas Shimoda Addition, Middle Park High School, Thompson School District
bond projects, Crossroads Buildings 1, 2, 14 and 16, CSU-BRB Lab Addition, Flex R &. D
Building #2. He expertly oversees all aspects of his projects including supervising several
employees on a jobsite, receiving delivery of material and coordinating work with other
subcontractors when needed on thejobsite. Adeptly handles requests by other supervisors in the
coordination of joint tasks. Bevan holds an active Colorado Master's license.

d. Written description of the proposed products/services
This proposal is based on drawings by OZ Architecture and Ligiitswitch Architectural dated March
2, 2011 with the following clarifications:
Inclusions
• We include storage.
• We include providing and installing light fixtures per Attachment AB. Counts for all fixtures to
be per Attachment AB.
• We include circuitry for all -allery lighting per circuits shown on the Liehtswitch drawings and
not the panel schedules nor the original drawings by ME Group.

Exclusions
• We exclude furnishing of a Pavment and Performance bond
• We exclude sales tax.
• We exclude electrical permit, utility company fees, and generator for temporary power.
• We exclude pads and formed concrete, concrete light pole bases, concrete encasement, concrete saw
cutting/removal/patching, access panels, wall finish patch/repair and painting, roof patching or sealing

• We exclude 1-IVAC controls wiring or conduit.
• We exclude Sound/Paging/Security/TV equipment, backboards, cable, cable installation, cable
terminations and head end equipment or terminations.
• We exclude PhonetData equipment, backboards, cable, cable installation, cable terminations and head
end equipment, racking or terminations.
• We exclude painting of anv hind.
• We exclude anv third party testing and commissioning.
• We exclude anv costs associated with engineers reviewing alternate or value engineered liahtina
fixtures and controls.
• We exclude commissioning of the lighting control system.
• We exclude installation of track heads.
• We exclude lighting control of anv kind.
• We exclude the touch screen and all controls. Per previous discussions, all controls to be done by
the AV Show controller provided and installed by others.
Our proposal is subject to the following clarifications
• This proposal may be withdrawn by us if not accepted within 90 days from the date of the bid.
• Progress payments to be made every 30 days.
All material is guaranteed to be as specified.. All work to be completed in a workmanlike manner according
to standard practices. Any alteration or deviation from above specifications involving extra cost will be
executed only upon written orders, and will become an extra charge over and above the estimate. Our
workers are fully covered by Work-man's Compensation Insurance
