In compliance with your Invitation to Bid dated 4/2 , 20 12 and subject to all
conditions thereof, the undersigned a (Corporation, Limited Liability Company,
Partnership, Joint Venture, or Sole Proprietor) authorized to do business in the
State of Colorado hereby proposes to furnish and do everything required by the
Contract Documents to which this refers for the construction of all items listed on
the following Bid Schedule or Bid Schedules.
The undersigned Bidder does hereby declare and stipulate that this proposal is
made in good faith, without collusion or connection with any other person or
persons Bidding for the same Work, and that it is made in pursuance of and
subject to all the terms and conditions of the Invitation to Bid and Instructions to
Bidders, the Agreement, the detailed Specifications, and the Drawings pertaining
to the Work to be done, all of which have been examined by the undersigned.
Accompanying this Bid is a certified or cashier's check or standard Bid bond in
thesum of four thousand twenty four & 75/100
($ 4024.75 ) in accordance with the Invitation To Bid and Instructions to
Bidders.
The undersigned Bidder agrees to execute the Agreement and a Performance
Bond and a Payment Bond for the amount of the total of this Bid within fifteen
(15) calendar days from the date when the written notice of the award of the
contract is delivered to him at the address given on this Bid. The name and
address of the corporate surety with which the Bidder proposes to furnish the

All the various phases of Work enumerated in the Contract Documents with their
individual jobs and overhead, whether specifically mentioned, included by
implication or appurtenant thereto, are to be performed by the CONTRACTOR
under one of the items listed in the Bid Schedule, irrespective of whether it is
named in said list.
6. Payment for Work performed will be in accordance with the Bid Schedule or Bid
Schedules subject to changes as provided in the Contract Documents.

Perform the work as detailed in the Avery bid #7360 and the addendum page.
If the bid is awarded on 4/9/12 we would like to get the fencing and the scaffold erected from 4/9/12-4/13/12.
We would start shingle tear off and chimney repair on 4/16/12.
We would like to finish on 5/22/12 and have the scaffolding and the fencing removed on 5/23/12.
Safety
1) All entrances and sidewalks shall be properly marked and secured for public safety.
2) A 6' high chain link fence with privacy screen shall be erected around the entire perimeter of the house.
3) 3) Scaffolding shall be erected around the entire perimeter of the house. The owner may want to take

Removal and decking
1) Remove all existing wood shingles down to the decking.
2) We would like to have the parking spots in front of the Avery house on Meldrum for the length of the
— project.
3) We will park our big dump trailer on the street and shuttle debris with tarps or small trailers as to not rut"

4) Recycle all wood shingles-at-Hagemens. Approximately 38 yards --"
-- 5) Replace any rotten decking.
-- 6) All decking shall be re=nailed because of age and -shrinkage. -_
7) Install wood inserts on the gable ends of the dormers between the sheathing and crown molding for
proper nailing.
8) Install plywood over spaced sheathing as needed.
9) Install cut to fit inserts over the windows for protection.
1-0) Protect the stonewalls with plywood.
- — -11) All debris will be cleaned up daily: — -
12) We will start on the NE corner of the building and work in a clockwise motion.
13) All sections will be dried in with I&W and felt the same day that they are torn off so as to keep a water
tight system at all times.
14) Some bat mitigation may be required during tear off. We will work with the proper authorities to make
sure this happens in a safe and humane manner.
15) We will run magnets around the property upon completion to retrieve any missed nails or staples.
Flashing and Underlayment
1) Install one course of Grace I&W on the eaves and valleys.
2) Install double layer 30# ASTM D 226 felt.
3) Install w valley and drip edge.
4) Install flashing and counter flashing on the chimneys.
5) Install new pipe jacks and heat stack jacks.
6) There are 4 applications where the roof meets a stone wall. These areas will need to be flashed and
counter flashed.
7) We will have a metal break on site to fabricate our flashing as needed.
8) R&R metal ridge.
Venting
1) R&R existing Rv 51 roof vents
2) Install cedar breather.
3) Install vented roll ridge on the ridges.
Shingles
1) Install smooth sawn, Class B, fire treated, 18" perfection shingles with a 5 %z" expose.
2) Use proper fasteners.
3) Shingles to be purchased by the owner.
— - Gutters--
1) R&R all gutters and downspouts.
2) Gutters and downspouts may contain lead paint. We will test the gutters and if needed remove the
gutters and downspouts with proper lead abetment techniques.
3) Gutters and downspouts to be recycled by the owner.
Painting
1) All painting to be performed by the owner.
Flat Roof
1) Install %z" cdx over the existing membrane.
2) Install fully adhered 60 mill white EPDM.
ALL work to be preformed as per specs.

SPECIFICATIONS AND CONTRACT DOCUMENTS
Description of BID 7360: Avery House Re -Roof
OPENING DATE: 3:00 PM (Our Clock) April 2, 2012
To all prospective bidders under the specifications and contract documents described
above, the following changestadditions are hereby made and detailed as follows:
• In order to expedite the project the City of Fort Collins will purchase the
cedar shingles. Do not include shingles in your bid.
• Shingle width is limited to 4" minimum to 10" maximum.
• Contractor to provide a 5 year minimum 'Water tightness" warrantee.
• Sheet flashing materials can be 26 gauge ASTM Berridge Galvalume or equal,
instead of 24 gauge.
• The existing copper flashing material on the turret can be replaced with either
galvanized metal or copper.
• Extend eave/ ice dam protection membrane 36" around the perimeter of the
building and one continuous 36" width sheet 18" each side of the valleys.
Addendum to item number 3.02 D.
• The existing wood cedar shingles shall be recycled at a site approved by the
City. Existing metal flashing shall be collected by contractor and the City will haul
off to City recycle bins. '
• Single ply membrane allowed on the flat roof section will be either white EPDM or
TPO roofing.

dewatering, overhead, profit, insurance, etc., to cover the complete Work in place of the

Bidder acknowledges that the OWNER has the right to delete items in the Bid or change
quantities at his sole discretion without affecting the Agreement or prices of any item so
long as the deletion or change does not exceed twenty-five percent (25%) of the total

as Principal, and as Surety, are hereby held and firmly bound unto the City of Fort
Collins, Colorado, as OWNER, in the sum of $*Ior the payment of which, well and truly

THE CONDITION of this obligation is such that whereas the Principal has submitted to
the City of Fort Collins, Colorado the accompanying Bid and hereby made a part hereof
to enter into a Construction Agreement for the construction of Fort Collins Project, .

(a) If said Bid shall be rejected, or
(b) If said Bid shall be accepted and the Principal shall execute and deliver a
Contract in the form of Contract attached hereto (properly completed in
accordance with said Bid) and shall furnish a BOND for his faithful performance
of said Contract, and for payment of all. persons performing labor or furnishing
materials in connection therewith, and shall in all other respects perform the
Agreement created by the acceptance of said Bid, then this obligation shall be
void; otherwise the same shall remain in force and effect, it being expressly
understood and agreed that the liability of the Surety for any and all claims
hereunder shall, in no event, exceed the penal amount of this obligation as
herein stated.
The Surety, for value received, hereby stipulates and agrees that the obligations of said
Surety and its BOND shall be in no way impaired or affected by any extension of the
time within which the OWNER may accept such Bid; and said Surety does hereby waive
notice of any such extension.
