(Waste Management) would like to thank you for allowing us the opportunity to submit a proposal for disposal of your UST Soils located in Ft. Collins, Colorado. We are confident that we can provide you with the highest quality of service and secure disposal of your non -hazardous industrial waste through the North Weld Landfill (NWLF) located in Ault, Colorado. Our commitment is ongoing and we will work to make sure our response precisely fits your needs. Scope of Work Includes: • Generator's waste profile sheet review of non -hazardous waste; • Secure disposal of non -hazardous industrial waste within NWLF Subtitle D Landfill, and • Preparation of manifests and/or labels. All loads must be accompanied with a signed completed manifest. Scope of Work Does Not Include: • Management of any waste regulated as hazardous waste as defined by the Resource Conservation & Recovery Act (RCRA); • Waste containing polychlorinated biphenyls (PCBs) regulated under the Toxic Substance Control Act (TSCA); • Management of any waste requiring a radioactive materials license, pursuant to a state or federal radioactive materials license program, radioactive source material (uranium or thorium greater than 0.05% by weight), or nuclear reactor and man-made radionuclides; • Secure disposal of friable and non -friable asbestos containing wastes; • Loading and unloading of waste containers; • Transportation activities; • Management of containerized waste; • Provision of truck liners; • Analytical services necessary for proper characterization of wastes, or • Any work outside the specified scope of work listed above. All rates quoted are based on this specific project and volume only. Quantities listed above are estimates only. Actual invoices will be based on actual quantities invoiced at the unit rates listed above, subject to minimum disposal volume of 3 tons/load. 2. Proposal is conditional on the disposal facility receiving all appropriate Waste Management internal and external regulatory approvals. 3. All pricing is conditional on the execution of a mutually acceptable contract. Standard Services and labor rates are not applicable for weekend and holiday work. 4. Proposed pricing presented in this document is valid for 30 days. All rate extensions must be agreed to in writing only. 5. Unit disposal pricing does not include fuel surcharge, environmental fee, or Hazardous Substance Response Fund (HSRF) Fee. a. The HSRF is a state imposed fee required on all waste deposited at all Colorado landfills. 6. Any required analytical is the responsibility of the generator. Analytical services can be subcontracted through Waste Management and quoted upon request. A Chain of Custody and complete and final analytical package is required with all analytical results. Waste Management can only accept analytical results from laboratory facilities that are certified by the appropriate regulatory agencies. 7. Any fees associated with the approval of the waste are the responsibility of the generator. A $75.00 profile approval and waste manifesting fee is assessed for each facility approval and each Special Waste Application submitted. Standard approval time is 48 hours, however, profiles can be rush approved for a $100.00 fee. Profile forms can be obtained at www.wmsolutions.com. Additionally, customers that are e- signature enabled can submit paperwork electronically. 8. Prices do not include transportation. Transportation can be quoted upon request. Generator/Customer is responsible for insuring all loads are loaded properly per DOT guidelines. Overweight fines are the responsibility of the Generator/Customer. 10. Proposed pricing becomes valid contract pricing upon acceptance by both Waste Management and the customer. The customer must indicate acceptance by initialing on the bottom of each page, signing the final page, and returning the document to Waste Management. Contract pricing represented in this document is valid for the term of the event or twelve (12) months, whichever is less. The special pricing represented in this document is based on delivery of all above -mentioned waste to the approved Waste Management facility. Delivery of above -mentioned waste to a facility other than a Waste Management facility may result in termination of this agreement or other actions as outlined in the terms and conditions of the current Industrial Service Agreement on file. 11. Additional special handling fees and definitions: a. Certificate of Disposal $25.00 -Available on profiled, manifested waste loads only. Request for certificate of disposal must be established and documented during the profile process. b. Manifest and / or label Printing Fee $1.00 per manifest, $25.00 minimum - Waste Management can supply pre-printed shipping documents for your special waste shipping needs. Add $25.00 if overnight mailing is required. Waste Management is confident that we can provide you with the highest quality of service.
