accordance with the terms and conditions contained herein and In the Services Agreement
between the parties. In the event of a conflict between or ambiguity in the terms of the Services
Agreement and this work order (Including the attached forms) the Services Agreement shall control.
The attached forms consisting offalte 6 pages are hereby accepted and incorporated herein by this
reference, and Notice to Proceed is hereby given.

Scone of Work
1. Remove existing slate roofs and replace with new Owens Corning Oakridge Dimensional shingles. (Color to be determined from the manufacturer's list
of standard, regional colors. Must initial color option at the bottom of this proposal.) This proposal includes for both slate roofs.
2. Install standard Owens Coming starter shingles.
3. Install new 309 felt underlayment.
4. Install new drip edge metal at all eaves.
S. The entire roof substrate of both slate roofs will be re -decked with new 1/2" 0511. (See Item #14 below
6. The slate roof will need to be removed. As much of it will be salvaged as possible, but no guarantee can be made.
7. New Ice and water shield will be installed at all perimeter edges Of the slate roofs.
8. The two existing 20' vents will be detached and reset as necessary. All stack flashings will be removed and replaced as necessary.
9. Ail roofing debris will be cleaned up daily. Every effort will be made to keep the site clean and presentable throughout the project.
10. Anticipated completion time is 3 days.
1L Due to the location of this structure, access is an Issue. This proposal Includes associated costs for the tear -off, disposal, and hand -loading of the roofs.
12. No permit or use tax is included in the price below.
13. Price includes all necessary labor, materials, licenses, deliveries, miscellaneous Rashings, sealants, and clean-up as may be required for a complete
installation as described herein. All work will be performed in a courteous, professional, and workmanlike manner.
14. It Is possible that the existing substrate may continue to dry rot under the OSB. RTN is covering the existing substrate with 1/2" OSB and is not
responsible for any deterioration caused by or to the existing substrate.
