HomeMy WebLinkAbout118332 TEAM FORT COLLINS - CONTRACT - PURCHASE ORDER - 9115130ACTUALITY" MEMORANDUM OF AGREEMENT This Memorandum of Agreement (hereinafter "MOA") is entered into between Team Fort Collins and The City of Fort Collins (hereinafter "Client'). The purpose of this MOA is to articulate the roles and responsibilities of Team Fort Collins and the Client in implementing a single cycle of an ACTUALITY° Social Norm Campaign at The City of Fort Collins. The initial cycle of an ACTUALITY® campaign begins with the signing of the MOA by all parties and concludes 30 days following the completion of the post -campaign report. The duration of the initial cycle will vary, according to the specific timeline of the project. Subsequent cycles require a new MCA. This MCA shall commence upon 8/25/11. 1. Training, Technical Assistance and Project Management Team Fort Collins agrees to provide • Training: Site Visit I —two trainers, %2 day • Training: Site Visit II —two trainers, %2 day [Trainings will take place on mutually agreed upon dates. Evaluation Based on the Client's selection of Option A, Team Fort Collins agrees to • Consult on survey questions to be added to existing Mayo Clinic Survey • Respond to specific questions by Client's local evaluator (not to exceed 2 hours of time on phone, email, etc.) • Provide analysis of Mayo Clinic's raw data Client agrees to • Adhere to suggested pre and post survey timeline • Provide pre and post -campaign raw data • Provide pre and post -campaign reports Fees Option A Evaluation 3. Progress Review $ 300 Total This Section $ 300 A Progress Review of the campaign will be initiated and conducted by Team Fort Collins on or about 8/2012. This is to help ensure client satisfaction and review the project timeline. 4. Warranties and Liabilities The client shall be fully responsible for implementing the social norm campaign created by Team Fort Collins and for meeting the associated outcomes and goals thereof. In no event will Team Fort Collins or its subcontractors be liable to the client or any third party for any damages, including any lost profits, lost savings or other incidental, consequential or special damages arising out of the client's operation of or inability to operate the social norms campaign, or the failure of the campaign to meet the stated goals, even if Team Fort Collins has been advised of the possibility of such damages. S. Termination of Contract Either Party may terminate this agreement with thirty days' written notice. If the agreement is terminated, Team Fort Collins will present Client with a statement of account showing all fees paid to that time, and itemizing work performed and costs for travel reservations that are not recoverable. If work performed exceeds fees paid to date, Client will pay Team Fort Collins for such work at the rate of $150 an hour. If travel costs that are not recoverable exceed fees paid to date, Client will pay Team Fort Collins for these as well. If fees paid exceed work performed to date, Team Fort Collins will return unearned fees to Client. 6. Payment for Services In consideration of the services performed, The City of Fort Collins will pay Team Fort Collins a total of $2,000 upon receipt of invoice. Acceptance of this Memorandum of Agreement This agreement is approved and accepted by all parties and is executed by the signatures of authorized representatives of all parties. This agreement has been reviewed and understood by all parties. Addendum: Evaluating an ACTUALITY® Social Norm Campaign The entire evaluation process from beginning (pre -campaign) to end (post -campaign) is as follows: • Develop pre -campaign survey • Data entry of pre -campaign data • Clean pre -campaign data to eliminate respondents not taking survey seriously • Analyze responses in pre -campaign data • Report on response percentages for the total population and by gender and by grade • Add post -campaign questions to survey • Data entry of post -campaign data • Merge post -campaign data into database that contains pre -campaign data • Clean post -campaign data to eliminate respondents not taking survey seriously • Analyze responses using analyses of variance to assess change on target issue • Report on ACTUALITY® Social Norm Campaign which includes the following: Recognizability of the campaign by the target audience Believability of campaign by the target audience Assessment of change on target issue You have elected Option A in the ACTUALITY®. Option A entitles you to the following: • Pre -campaign survey development • Availability of ACTUALITY® evaluator to respond to specific questions by your local evaluator (not to exceed 2 hours of time on phone, email, etc.) • Addition of post -campaign questions to survey • Analysis of raw data as provided by Mayo Clinic Upon completion of your ACTUALITY® Social Norm Campaign, a spreadsheet containing pre- and post - campaign data should be sent to Dawn Nannini at dawn@teamfortcollins.org. This is only to ensure the fidelity of the ACTUALITY® process. Data will not be used for presentation or publication without prior permission.
