RE: North College Phase II Improvements- Vine to Conifer
Final Engineering Contract Modification — Scope Amendment for Pavement Section
Dear Kyle:
At the request of the City, the pavement section for North College Avenue has been modified for
the North College Phase II Improvements — Vine to Conifer. The original section included (top
to bottom):
2" hot mix asphalt (HMA)
• 1" HMA Reflective Crack Interlayer
• 8" concrete pavement
•6" aggregate base course (ABC)
24" overexcavation (swell mitigated subgrade)-
The City has decided to change the approach to subgrade stabilization. The new pavement
section includes (top to bottom0:
• 2" HMA
1" HMA Reflective Crack Interlayer
8" concrete pavement
24" ABC
• geogrid
Accordingly, the plans, cross -sections, and quantities require revision. Stolfus & Associates,
Inc. will revise the affected sheets with the new pavement section and incorporate the into the
Construction Documents. This scope amendment includes typical section revisions; roadway
modeling; cross-section revisions; updated quantity calculations for earthwork, ABC, and
geogrid; tabulation revisions; summary of approximate quantity updates; and opinion of
probable cost updates. The following plans will be revised:
• Typical Sections (4 sheets)
• Cross -sections (54 sheets)
• Tabulations (Earthwork and Surfacing)
• Summary of Approximate Quantities
This scope also includes project management and coordination required to complete the

revisions including day-to-day coordination via phone and e-mail, one conference call, and one
meeting with the City, as necessary.
Based on these modifications, we respectfully request a Contract modification of $13,360 with a
revised "Not to Exceed" limit for the project of $411,288. Please accept this letter as a
modification to the original Contract.

(Final Right -of -Way Plan Revisions)
Dear Michelle,
The following scopeof work and associated fees are to provide Revisions to the existing Right -of -Way plans, legal descriptions,
and exhibits for the North College Right -of -Way Plans based upon the conversations and correspondence between Farnsworth
and yourself. Our scope of work and fees are as follows:
Scope of Work:
Make the following changes on the ROW plans:
• Update Owners
o Parcel 106— Owner address change
o Parcel 108—New Owner
o Parcel 109—New Owner
o Parcel 128 - Owner address change
o Parcel 132 - Owner address change
o Parcel 133 - Owner address change
• Parcel 114
o Update ROW and TCE linework to not include the overhang at Dingo Planet
• Parcel 117
o Update the TE so the eastern building face is not included in the TE
• Parcel 137
o Change to a total take

Survey Control Diagram:
Farnsworth will make the requested revisions and include the Land Survey Control Diagram in the revised Right -of -
Way Plan set
Right -of -Way Plans/legal descriptions, and exhibits:
Farnsworth will make the noted changes using information supplied by the City of Fort Collins and will re -submit the
revised documents.
Estimated Fees:
Farnsworth Group will perform the above -noted Services on an hourly basis not to exceed $7,088, to be billed in
accordance with our current billing schedule for this project.
Schedule:
We will begin immediately upon "Notice to Proceed".
