Scope of Services: Additional 17 days of excavation oversight and materials management for the storm line, North _ grading and conduit, final grading after trailer remeval and the monument sign activities at the Discovery Museum site, plus required reporting for the current phase and July 2010-Apri12011, per your Proposal dated May 31, 2011. Service Provider agrees to perform the services identified above and on the attached forms in accordance with the terms and conditions contained herein and in the Services Agreement between the parties. In the event of a conflict between or ambiguity in the terms of. the Services Agreement and this work order (including the attached forms) the Services Agreement shall control. At your request, WALSH Environmental Scientists and Engineers, LLC (WALSH) is pleased to provide this request for contract value increase for excavation oversight and materials management services for the Discovery Museum project. This document provides a summary of estimated project -related costs based on a soil disturbing schedule of seventeen (17) days for the phases identified above. This document also includes estimated costs to complete required project reporting functions for the period of July 2010 through April 2011. A spreadsheet identifying estimated charges is included as Attachment A. WALSH respectfully requests a contract value increase for this phase of the project in the amount of $33,978 which includes a ten (10) percent contingency on labor hours. Actual project costs may be more or less depending on the final schedule, actual site conditions encountered, and available personnel assigned to the project. Labor and other direct costs will be billed on a time and materials basis according to established 2011 rates identified in WALSH's General Environmental Services Contract (P 1066) with the City. This proposal assumes a standard eight (8) hour work day on site with additional time [one (1) hour] allotted for field equipment calibration, maintenance, etc. for a total of nine (9) hours daily. For daily oversight consisting of four (4) hours or less per day, a half day rate will be charged. For daily oversight consisting of more than four (4) hours but less than or equal to eight (8) hours per day, a full day rate will be charged. A half day rate will be charged for work cancellations (i.e., due to weather conditions, scheduling conflicts, etc.) where notice is received less than the end of business the day in advance. It should be noted that asbestos soil inspection is considered a limited screening activity conducted to comply with the requirements of the MMP, SCMP, and State regulations, but is not exhaustive and cannot guarantee or warranty soil to be free of asbestos debris or contamination.
