Since individual Survey Project Work Orders have been negotiated with both Project Managers Zoe Whyman and
Lucinda Smith, the City is ready to proceed with the Services Agreement.
Attached is the Services Agreement file for RFP# 7226 for Survey Services - Miscellaneous Public Opinion
Surveys. Please review, print 3 copies, sign, and return all 3 to my attention for City's signatures along with
required Certificate of Liability Insurance (per Section 18) naming the City as additional insured.
