PROJECT DESCRIPTION
This scope of work is for the emergency bridge inspection and related services of structure numbers OAK-
MTN-A and OLIV-OAK-A. This work order is performed trader the master agreement for the City's
Bridge Inspection, Maintenance, Repair, and Replacement Program.
SCOPE OF SERVICES:
This scope of work is for the emergency bridge inspection and related services of stricture numbers OAK-
MTN-A and OLIV-OAK-A.
OAI{-MTN-A
It was discovered by the City that a sink hole was formed in the alley near the outer extents of
Arthur Ditch. The source of the sink hole is unknown and located within the City's Right.of.
Way. Since Arthur Ditch is scheduled to run water within the second week of April, it is an
urgent matter to find the cause of the sink hole. Balser is tasked to provide the City of Fort Collins an
Emergency Bridge Inspection, provide a reassessment of the structure's condition, provide an updated
Sufficiency Rating, and analyze the cause of a sink. hole that has formed adjacent to the structure. In
addition, Baker can provide on construction support for the immediate repairs made by the City's wort.

Work elements include: Project administration, field inspection, coordination with the City regarding
immediate findings and to institute immediate repairs, photos and measurements, reporting, documentation
of the process through a memorandum, and coordination with the City regarding the recommended course
of action. A load rating will not be required.

OL IV -OAK -A
The City has a sanitary sewer pipe that crosses underneath this Arthur Ditch structure, running parallel to
the alley. It was discovered by the City that the amount of influent water being processed at the City's
water treatment facility drastically increases when Arthur Ditch runs water. Processing the increased
influent from Arthur Ditch is costly for the City and unnecessary if the excess water can be properly
contained to Arthur Ditch. There is suspicion that a leak in the ditch structure is the cause of the extra
influent to the sanitary system. Baker's task is to provide the City of Fort Collins with an Emergency
Bridge Inspection for the purpose of locating any voids, cracks or holes that would allow water to flow out
of Arthur Ditch and into the City's sewer system.
Work elements include: Project administration, field inspection, coordination with the City regarding
immediate findings and to institute immediate repairs, photos and measurements, reporting, documentation
of the process through a memorandum, and coordination with the City regarding the recommended course
of action. A load rating will not be required.
GUIDELINES AND REFERENCES
All inspections will be conducted in accordance with current NBIS standards and supervised by personnel
meeting the requirements outlined in the Code of Federal Regulations, 23 CFR 650.307. Applicable
inspection standards include:
• CDOT PONTIS Bridge Inspection Coding Guide
• Recording and Coding Guide for the Structure Inventory and Appraisal of the Nation's
Bridges (Report No. FHWA-PD-96-001)
• Bridge Inspector's Reference Manual (Publication No. FHWA NHI 03.001)
• AASHTO Manual for Bridge Evaluation
• CDOT Bridge Rating Manual
• Any other applicable documents as approved by the City
DELIVERABLES
The following deliverables will be provided for this project.
1. Project Administration
• One progress report with invoice
• Quality Control review of all project deliverables prior to submittal
2. Inspection
• One printed copy of bridge inspection reports (rating form and photos in color)
• One PDF file of the bridge inspection report for each bridge inspected, including a one -
page summary sheet indicating "Urgent", "Routine", and "Safety Repairs Required",
with costs and sufficiency rating
• All photos in a digital format with descriptions
• Electronic sketches of the bridge in a Microstation or AutoCad format
• PONTIS output of six (6) tables in excel format (bridge, eleminsp, insp_weand,
inspevnt, roadway, and userbrdg)
• Recommendations for re -rating of bridges, if any

Challenges
Public Perception: The recording of video in public places is completely legal and
has become extremely common. However, there may be parties who perceive that
the video recording of their actions is distasteful and may react negatively to the
video system.
Operation and Maintenance: Fort Collins Police Services is pleased to assist with
the design and installation of a video surveillance system in Old Town Square as a
public safety benefit. However, Fort Collins Police Services will not be able to
maintain the system as they are bound by rules of evidence that would make the
system extremely cumbersome to operate. The DDA as a quasi governmental agency
is bound under many of the same standards. A separate private entity will need to
maintain ownership of the system and house the central data station (DBA). It is
recommended that the recording system be purged after a pre -determined number of
days to allow reasonable requests for video recordings, but also prevent requests from
becoming frivolous or burdensome. All stakeholders will have access to the system
via live intemet viewing. If they wish to view historical video, or make recordings,
they will need to come to the DBA office and access the main video terminal during
normal business hours. A standard request protocol will need to be developed for non
stakeholders who wish to view or record historical video.
Costs
After researching several options, First Class Security Systems seems to provide the
highest quality system at a reasonable price point. The system will be "turn key" in
that all camera and digital video recording functions will be operational at project
completion. However, there may be additional costs from an IT perspective to
incorporate the live video feed onto the Downtown Fort Collins web site.
The initial bid from First Class Security for three cameras on the west side of the
square with no major installation complications is $9769.16. **See Attached**
