DATED: April 13, 2011 Work Order No. 400902100-MB-3-EI02 Purchase Order No. Project title: Bridge Inspection, Maintenance, Repair and Replacement Program Emergency Inspection of Str. No. FCMULB-CRESTMR Commencement Date: March 25, 2611 Completion Date: May 31, 2011 Maximum Fee: $5,000.00 PROJECT DESCRIPTION This scope of work is for the emergency bridge inspection and related services of structure number FCMULB-CRESTMR. This work order is performed under the master agreement for the City's Bridge Inspection, Maintenance, Repair, and Replacement Program. SCOPE OF SERVICES: This scope of work is for the emergency bridge inspection and related services of structure number FCMULB-CRESTMR. During a recent field inspection by the City's major bridge inspection consultant, it was determined that the condition of this bridge was compromised to the point that emergency action was required. The City decided place a load posting sign along Mulberry Road for an indefinite duration. Baker. is tasked with providing a secondary opinion for the condition and safety, as well as for the recommended course of action. Work elements include: Project administration, field inspection, coordination with the City regarding immediate findings, photos and measurements, reporting, documentation of the process through a memorandum, and coordination with the City regarding the recommended course of action. No, FCMULB.CRE5rMR GUIDELINES AND REFERENCES All inspections will be conducted in accordance with current NBIS standards and supervised by personnel meeting the requirements outlined in the Code of Federal Regulations, 23 CFR 650.307. Applicable inspection standards include: • CDOT PONTIS Bridge Inspection Coding Guide • Recording and Coding Guide for the Structure Inventory and Appraisal of the Nation's Bridges (Report No. FHWA-PD-96-001) • Bridge Inspector's Reference Manual (Publication No. FHWA NHI 03.001) • AASHTO Manual for Bridge Evaluation • CDOT Bridge Rating Manual • Any other applicable documents as approved by the City DELIVERABLES The following deliverables will be provided for this project. 1. Project Administration • One progress report with invoice • Quality Control review of all project deliverables prior to submittal 2. Inspection • One printed copy of bridge inspection reports (rating form and photos in color) • One PDF file of the bridge inspection report for each bridge inspected, including a one - page summary sheet indicating "Urgent", "Routine", and "Safety Repairs Required", with costs and sufficiency rating • All photos in a digital format with descriptions • Electronic sketches of the bridge in a Microstation or AutoCad format • PONTIS output of six (6) tables in excel format (bridge, eleminsp, insp_wcand, inspevnt, roadway, and userbrdg) • Recommendations for re -rating of bridges, if any SCHEDULE All work shall be completed by May 31, 2011, April A 2011 2 of
