Project Description:
Walsh Environmental Scientists and Engineers, LLC (WALSH) will prepare an addendum to the Soil Characterization
and Management Plan for the Poudre River/Arilan Site in Fort Collins, Colorado (SCMP).
Scope of Services: The scope of services Includes the following:
• Most or teleconference with assigned City of Fort Collins, Colorado State University, Colorado State University
Research Foundation, and Colorado Department of Public Health and Environment (CDPHE) representatives to
discuss applicability of State regulations to project, define specific projects goals, and discuss proposed
modifications to SCMP.
• Prepare letter addendum defining scope of project, work practices, and generals updated graphlcsl8gures.
• Submit electronic (Adobe - Portable Document Formal) version of SCMP addendum to CDPHE for technical
review/approval.
• If necessary, discuss SCMP addendum with CDPHE and City of Fort Collins representatives and Incorporate any
requested changes.
• Submit electronic (Adobe - Portable Document Format) version of final SCMP addendum to CDPHE for technical
review/approval.
• Upon receipt of SCMP addendum approval letter from CDPHE, submit electronic version and two hard copies of
the final SCMP addendum to CDPHE and City of Fort Collins representatives. Submit two hard copies of the final
SCMP addendum to CDPHE (Voluntary Ciean-up Program representatives) as necessary per existing
requirements and agreements.
• Estimated costs do not inciudefees for providing soil disturbing oversight/managemard, alrmonloring, or analytical
services.
Professional agrees to perform the services identified above and on the attached forms in accordance with the terms and
conditions contained herein and In the Professional Services Agreement between the parties. In the event of a conflict
between or ambiguity In the terms of the Professional Services Agreement and this work order (including the attached
forma) the Professional Services Agreement shall control.
The attached forms consisting of consisting of (1) page(s) (4/26/2011 Cost Estimate Spreadsheet) are hereby accepted
and Incorporated herein, by this reference, and Notice to Proceed Is hereby given.
