Re: Bid 7120, Hauling, Determination of Responsiveness and Responsibility of Bidder
Dear Mr. Winick,
The City of Fort Collins ("City") appreciates SJW Trucking's interest in the contract for
the above -referenced project. The City has conducted a review of your submittal to
determine whether it meets the responsiveness and responsibility criteria of the Code of
the City of Fort Collins ("City Code").
Your bid included only your CDL Drivers license. One truck at the $58.00 per hour will
be allowed to work for the City with you as the driver. Any change in drivers will require
that you provide the drivers CDL license and workman's compensation or proof that you
do not need workman's compensation.
ATTENTION: SPECIAL INSTRUCTIONS
The City has special procedures for signing the Service Agreement and acquiring the
insurance certificate's. The Service Agreement that is included with the bid
documents will become your contract. Please sign it. If any vendor wants a copy of
below. The City is requiring the following to be included with your bid:
1) Insurance certificate naming the City of Fort Collins as an additional insured
and workman's compensation as needed.
2) Copy of alcohol and controlled substances testing program.
3) Copy of CDL Drivers license for each driver.
4) E-mail address or cell phone number.
5) Your bid amount in the bid schedule. This will become your Exhibit "C" in the
Service Agreement.
6) Please sign the Service Agreement and date it.

All requirements must be submitted with your bid. The City reserves the right to not
add any service provider to the hauling list if not complete.
