HomeMy WebLinkAboutWORK ORDER - RFP - 7192 PERFORMANCE EXCELLENCE CONSULTANTWORK ORDER #1 EXHIBIT "A" WORK ORDER FORM PURSUANT TO AN AGREEMENT BETWEEN THE CITY OF FORT COLLINS AND CENTER FOR PERFORMANCE EXCELLENCE DATED: February 15, 2011 Work Order Number: 1 Purchase Order Number: Project Title: 7192 PERFORMANCE EXCELLENCE CONSULTANT Commencement Date: February 15, 2011 Completion Date: May 31, 2011 Maximum Fee: (time and reimbursable direct costs): $44,750.00 Project Description: As stated in Section 1: Scope of Services PAGE 1 OF 4 Scope of Services: Provide assistance in implementing a performance excellence program for City of Fort Collins operations: identify and prioritize areas for improvement in performance excellence program; provide assistance to writing in developing an effective Foothills Level application for consideration by Colorado Center for Performance Excellence (CPEx) in 2011 (see www.coloradoexcellence.org for additional information regarding the Foothills Level): focus on developing a sound "approach" to the performance excellence program which will lay the groundwork for program work after completion of the Foothills Level application: train staff and consult on effectively implementing business practices consistent with Baldrige criteria for performance excellence: and advise on strategic planning process for entire City organization. Professional agrees to perform the services identified above and on the attached forms in accordance with the terms and conditions contained herein and in the Professional Services Agreement between the parties. In the event of a conflict between or ambiguity in the terms of the Professional Services Agreement and this work order (including the attached forms) the Professional Services Agreement shall control. The attached forms consisting of three ( 3 ) pages are hereby accepted and incorporated herein, by this reference, and Notice to Proceed is hereby given. Support strategic planning process consistent with Performance $13,750 Excellence Criteria. Advise the writing Team on completion of the 2011 Foothills $10,000 application of CPEx. Review and comment on draft Foothills applications. $ 6,000 Total $44,750 Charges include: • One consultant per class training and work session; • Administrative support for scheduling, class preparation, development of training materials specific to the City's needs and monthly reporting of progress; • Copies of handouts for training; • Materials needed for work sessions; • Local travel for consultants and • Meeting room at the Center for Performance Excellence.
