Project Description: As stated in Section 1: Scope of Services

Scope of Services: Provide assistance in implementing a performance excellence program for City of Fort
Collins operations: identify and prioritize areas for improvement in performance excellence program; provide
assistance to writing in developing an effective Foothills Level application for consideration by Colorado
Center for Performance Excellence (CPEx) in 2011 (see www.coloradoexcellence.org for additional
information regarding the Foothills Level): focus on developing a sound "approach" to the performance
excellence program which will lay the groundwork for program work after completion of the Foothills Level
application: train staff and consult on effectively implementing business practices consistent with Baldrige
criteria for performance excellence: and advise on strategic planning process for entire City organization.
Professional agrees to perform the services
identified above and on the attached forms in
accordance with the terms and conditions contained
herein and in the Professional Services Agreement
between the parties. In the event of a conflict
between or ambiguity in the terms of the
Professional Services Agreement and this work
order (including the attached forms) the Professional
Services Agreement shall control.
The attached forms consisting of three ( 3 ) pages
are hereby accepted and incorporated herein, by this
reference, and Notice to Proceed is hereby given.

Charges do not include meals or refreshments for training and work sessions.
