This pricing helps simplify what is a usually considered a very complicated product in that every pair of eyewear is manufactured for one individual patient with individual specifics unique to that order. Over the years this pricing method has been embraced and understood - we would recommend continuing to offer the package pricing strategy. Conversely if the City would like to explore the "a -la - carte" methodology we will be happy to revisit the pricing. (e.) Dispensing: Dispensing will be handled off -site. I have enclosed a detailed list of current Eye Care Providers in the Fort Collins area. Please visit our web site, www.aosafetysrx.com and click on the "Professional Eye Care Locator" tool. Should additional off -site dispensing locations be required, 3M Company will coordinate efforts to add such dispensing resources and provide a fitting kit of frame samples to the ECP at no charge to the City of Fort Collins. In this dispensing method, the opticians are capable of accurately taking the required optical measurements, assisting with frame and lens selection, and fitting and delivering the finished eyewear to your employees. The current negotiated rate is $ 22.00 per pair. This cost is billed as a separate line item on the invoice. (f.) Pricing: The attached price list illustrates our material quote for the City of Fort Collins. Prices are quoted firm and fixed for a two-year period with an option for two (2) - one-year renewals based on price renegotiation. Freight is included in all pricing. • • Value Added Features: Profile Order Forms: All program details such as products allowed and not allowed and co -pay options (if applicable) are shown on each form. This eliminates the guesswork typical with safety prescription eyewear programs and helps reduce the possibility of employees obtaining items not approved. Our Customer Service team verifies each order during the order entry process to ensure accurate invoicing and order processing. The profile order forms are location specific. On-line rules, a customized frame catalog, eligibility management and a custom ECP panel are some of the available tools that are provided at no cost. Invoicing Options: • In addition to traditional invoicing, we now offer customized EXCEL billing options. This permits detailed information to be provided in a sort -able format that can help with accounting and budgeting issues. Print on Demad POD forms: POD forms is a stand alone tool that can is provided as a desk -top hyperlink for your prescription eyewear coordinators. This link accesses a live database that populates an order form with current company information. This means the order forms are never out of date. In the event any aspect of the program changes, that change shows up on the form the very next day. WARRANTY 3M Company will replace any defective products at no charge if the following conditions occur within one year from date of purchase: • Lens breakage (not a warranty against scratches) ■ Frame breakage • Coating failure (peeling or crazing) ■ Corrosion of any frame component Warranties apply provided that no abnormal or excessive abuse is evident. Product must be returned to 3M. -tended Warranty: 3M frames in Group G and G PLUS are guaranteed for 2 years subject to the conditions above. POLICIES & PROCEDURES 3M NO-FAULT POLICY 3M will process a change with a simple telephone call from the Eye Care Professional. This policy expedites the reorder process and ensures that the eye glass wearer receives their replacement eyewear promptly. PRESCRIPTION CHANGES Any request for changes to a prescription, due to a doctor's or optician's change, will be processed according to the following policy: 1) 3M will replace any prescription or order change at no charge if the change is equal to or less than the original. A request for a more expensive replacement will require a charge for the difference in price. Subsequent prescription changes or order changes for the same eye glass wearer will be charged at 100%. 2) 3M will charge 50% on any prescription change or optician change request after 60 days from purchase. A limited one-year scratch warranty is offered on SuperCote, AR with SuperCote and AR SuperCote PLUS. Claims limited to one replacement per order. A one-year Warranty is offered on DX Rx Anti -Fog coating. DX Rx is compatible with plastic or polycarbonate only. It is not compatible with other lens materials, coatings or tints and is not offered with AR, SuperCote, Mirrors or Intimidator lenses. If the eye glass wearer is unable to adapt to their PALS, 3M Will replace the lenses with any multifocal lens at no charge within 60 days from date of purchase. After 60 days replacement lenses will be charged at full price. 3) Any request after six months from purchase date will be subject to an invoice for full price. Any request for an order change due to an eye glass wearer's change in preference Will be charged at 50% of the original invoice price for tip to 60 days, and at fill price thereafter. A request for a more expensive item will require a charge for the difference in price. PRESCRIPTION LENS REPLACEMENT Unless prohibited by state law, 3M will process an order for eyewear Where the prescription is less than two years old. We urge our customers to visit their Eye Care Professional to ensure that their prescription is accurate and current. REDOS Redos (Remakes) must be returned to the laboratory (address listed below). For fabrication errors, doctor's prescription changes, optician's changes and customer changes, please telephone 3M's Customer Service Center to obtain a RMA number (Return Material Authorization).
