Thank you for the opportunity to provide this proposal highlighting our ability and experience in
Chemical Spill Cleanups throughout Colorado.
Since 1997, BELFOR Environmental has provided quality Hazardous Material Response
services to our clients throughout Colorado. Our team of experienced professionals has built a
solid reputation and we are eager to continue working with the City of Fort Collins for your next
contract period to manage Hazardous chemical spill cleanups.
Currently, BELFOR Environmental maintains relationships with other municipalities. Some of
those clients have utilized BELFOR Environmental for both HazMat and Asbestos experience,
Our roots in Emergency Response provide an element of time sensitivity, that ensures that our
response team works diligently and safely at your project site.
Although BELFOR Environmental is providing our qualifications for our Chemical Spill cleanups,
our experience and capabilities go much further. We are part of BELFOR USA, the largest Fire
and ,W.Iter disaster recovery firm in'the United States. Though we are separate entities, we
maintain the ability to work together on catastrophic cleanups if such a need was to present
itself. An example of this was our response to the Katrina Hurricane in 2005 and Memphis
Floods earlier this year.
Since our last proposal offering, BELFOR Environmental has secured the licensing, opened
more offices and divisions to better manage large projects across the United States- without
depleting the core resources necessary to effectively manage local projects.

BELFOR Environmental has developed a rigorous training program to address the requirements of our
current contracts and are currently using this program to train employees and subcontractors. We
provide basic training and specialized training to our employees. Specific training is mandatory for
each position with other specialty training preferred. Training is mandatory for specific functions or
tasks and is mandatory as necessary. We also require corrective action training should deficiencies in
field or contractual performance be identified.
A BASIC TRAINING
Our basic training is given to all BELFOR Environmental employees and specific subcontractor personnel
who's job descriptions, responsibilities or authorities require it. This training focuses on health and safety,
contaminant specific, transportation and disposal and contract management functions.
Basic training requirements for particular types of employees and subcontractors vary according to job
descriptions. All employees and subcontractors are required to have current OSHA Health and Safety
i training. BELFOR Environmental personnel responsible for providing core services must have appropriate
contract administration and project management training. Specifically, our employees receive the following

Company orientation training is provided to initiate the employee to the company policies, procedures and objectives. A
four-hour block of instruction is provided by the immediate supervisor of each new BELFOR Environmental employee.
Company orientation training addresses:

OSHA 29 CFR 1910.120 Initial Training (40-hour) is provided (HAZWOPER training) to all personnel prior to their
assignment to sites containing known or unknown hazardous materials. This course provides personnel with basic
knowledge in site health and safety so they may work on a hazardous waste site. The course meets all requirements of 29
CFR 1910.120. Specific modules address:
• Toxicology
• Flammables
• Corrosives
• Reactives
• Shipping regulations
• U.S. DOT and OSHA requirements
• Protective clothing
• Respiratory protection
• U.S. EPA's levels of protection
• Hazard classifications

• Medical monitoring

• Site assessment and layout
• Site health and safety plans (HASPs)
• Contingency plans

• Air monitoring instruments
Our initial OSHA 40-hour course also covers U.S. EPA Orders 1440.2 and 1440.3 (respectively titled "Health and Safety
Requirements for Personnel Engaged in Field Activities" and "Respiratory Protection") and topics and exercises outlined in
the U.S. EPA course "Hazardous Material Incident Response Operations." These topics include:
• Chemical hazard evaluation through the use of reference materials
• Methods for selecting, maintaining and decontaminating protective equipment
• Evaluation of air monitoring instrumentation outputs
• Hazards and precautions associated with working in hot and cold environments
• Use and interpretation of a site -specific health and safety plans (HASPs)
OSHA 29 CFR 1910.120 Refresher Course (eight hour) satisfies the requirements of 29 CFR 1910.120 that hazardous
waste workers and supervisors must receive eight hours of refresher training each year. All field personnel receive this

training annually. The course updates their knowledge of health and safety practices and serves as motivation to work
safely. The course is designed to reinforce and update current health and safety practices. Training is provided for the
following modules:
• Regulatory reviews and updates
• Hazard recognition
• Respiratory protection
• Personal protective equipment
• Spill response procedures and equipment
• Site control and review of the previous year's incidents
OSHA Supervisor Training (eight hour) provides the specialized training required by OSHA for supervisors responsible for
the health and safety of personnel at hazardous waste sites. All BELFOR Environmental personnel who have supervisory
responsibilities receive this training prior to their assignment to a hazardous waste site. The eight -hour course addressees
the following modules:
• Regulatory compliance issues
• Implementation of health and safety plans
• Health hazard assessment
• Site management
• Accident prevention and liability
• Responding to emergencies
OSHA 40 CFR 763 Asbestos 54ay Contractor/Supervisor Training & 4-day Worker Training is provided to field and
supervisory personnel that respond to releases or abate friable asbestos containing materials. This training is conducted
through accredited training programs and meets the requirements set forth in 40 CFR 763 and Colorado Regulation
Number 8. Initial information presented during classes include regulatory, health effects, medical surveillance programs,
pre -work activities, insurance/legal issues, contract specifications and respiratory/personal protective. The operational
phase of abatement includes hands-on training methods for preparing and establishing a decontamination unit, methods
of air and bulk asbestos sampling, controlling airborne fibers and glovebag abatement methods. Post -abatement subjects
include encapsulation, cleanup, and proper transport and disposal.
Spill Response Operations and Equipment training is provided to field personnel to gain familiarity with BELFOR
Environmental's specific spill response resources, methods and client -specific requirements. Our primary response
equipment is demonstrated in a class -room environment as well as actual field demonstration. The demonstration is
hands-on by all attendees. The equipment's specific application, care and use is identified. Equipment includes
respirators/supplied air systems, pumps, skim mers,.containment boom and decontamination equipment. Client -specific
training is provided to brief our employees on specific issues regarding our inclusion and related responsibilities within
Client SPCC plans. This training is provided initially, and new equipment and procedures are identified during the OSHA 8-
hour Refresher Training,
OSHA 29 CFR 1910.146(g) Confined Space Entry and Rescue is provided to all personnel that enter or supervise
operations that require entry into a confined space. This training program includes a combination of classroom and field
exercise. The classroom training fully educates our personnel on BELFOR Environmental Confined Space Entry policies and
OSHA requirements, including permit requirements, space classifications, monitoring, engineering controls, rescue
equipment and procedures and specific responsibilities of personnel. The exercise provides hands-on confined space entry
and rescue trainingthrough a "mock" confined space. This program fulfills all requirements to qualify personnel for entry,
rescue, attendant and safety watch.

Inorganic and Organic releases.
Our Management personnel have decades of experience and routinely manage projects of all
sizes throughout the United States and more specifically the Rocky Mountain Region.
Our constant exposure to chemical hazards ensures you that the personnel are familiar with the
PPE requirements and safe handling techniques for managing your Chemical Spill Clean Up
and Hazardous Materials Handling Services.
BELFOR Environmental makes every effort to adhere to all applicable laws and regulations.
Our personnel have even been involved with the development of Methamphetamine regulations
in the state of Colorado and are experts in the field of HazMat Response.
Initial Response:
BELFOR has an excellent record for responding to emergencies throughout Colorado. In an
effort to maintain balance, BELFOR maintains 3 teams of HazMat Responders throughout the
month. Each team is led by a Senior Response Manager that receives all calls'that come in
through our 24/7 live answering service. In instances requiring immediate dialogue, .the
incoming call from your Emergency Personnel can be directly patched through to the Senior
Response Manager. Often that same Response Manager will go directly to the site ahead of the
team in order to get a direct understanding of a potentially complex situation'.
Our Teams are trained in and are fluent with NIMS and will fold into the response effectively by
conducting a safety briefing with response personnel. They will exercise all necessary
precautions to mitigate the hazard and ensure a safe work environment through ;their training on
proper site specific precautions though use of PPE and instrumentation that is documented for
future use.
All BELFOR Field personnel are fluent in completion of Daily Work Reports, and other
recordkeeping documentation — including photographs. This package is assembled and offered
to the client for their incident records.
Finally, BELFOR Environmental does not speak to the media, as a policy. Media is directed to
the client- as a matter of course.
Response and Project Work:
Attached are cost schedules for Scheduled and Emergency work. Response time to the actual
site is dependent upon variables such as: Location and Time of Day. As mentioned previously,
BELFOR Environmental makes every effort to be "out the door' within- one- hour, generally well
within that hour.
For scheduled services, we flexible in offering Time and Materials Bids, Fixed price and Unit
cost bidding. Often, we will value engineer a project together with a client to develop the best
solution to meet their needs.

CPR and First Aid Training is provided to BELFOR Environmental field personnel. Each employee receives periodic training
in cardiopulmonary resuscitation (CPR) and First Aid so if an emergency occurs they are prepared to administer life-saving
care until professional medical assistance arrives.
U.S. DOT HM181 and HM126F Training is provided to all BELFOR Environmental personnel involved with on -site handling
and preparation of hazardous wastes for shipment off -site. This training familiarizes personnel with U.S. DOT regulations
that govern preparation and shipment of hazardous materials. HM181 and HM126F training covers the following issues:
• Overview of U.S. DOT regulations
• Relationship to RCRA and CERCLA
• Manifesting
• Packaging and containers
• Labeling and placarding
Lead Awareness is provided to field and supervisory personnel that remove or encapsulate lead -containing materials.
This awareness training presents Lead standards, general operations typically encountered by BELFOR Environmental,
respiratory protection, medical surveillance, engineering controls and work practices, personnel protection and safety and
employee responsibilities. Our established compliance plan is also reviewed. This information is further presented prior to
specific job assignments.
Specialized Training
Specialized training is required for employees, team and non -team subcontractor personnel working on project sites that
present special challenges. Specialized training is tailored to meet job -specific hazards. BELFOR Environmental's
Operations Managers identify needs for specialized training as they develop. Special hazard training allows personnel to
perform hazardous tasks safely, in compliance with all applicable regulations that require specialized training. These

As always, Health and Safety of our personnel is of utmost importance. All BELFOR personnel
undergo a detailed yearly physical in addition to their entrance and exit physicals. They are
trained to meet or exceed any State or Federal guidelines for health and Safety protocols.
Those individual records are confidential, but a training summary is attached.
To ensure the safe execution of work at your project sites, a site specific health and safety plan
is prepared prior to commencement of the project. Daily Tailgate safety meetings are held to
address site specific hazards that may arise and each worker signs off on that document.
BELFOR has an excellent track record for working with the client to make sure the job is done
right the first time. Our Site Supervisors will work closely with Fort Collins personnel to evaluate
site conditions, determine the best disposal option and provide the associated paperwork to ship
and track that waste material. Where required, NFA letters will be obtained for the City of Fort
Collins.
Project Completion:
The BELFOR Project Manager is responsible for the closeout of the project and that person will
conduct debriefing with the City of Fort Collins assigned representative. Our billing package is
well received by our clients and it provides copies of all pertinent Daily Work Reports,
transportation and disposal documents in addition to the itemized bill for services provided. For

1. Personnel and Licenses-
BELFOR Environmental has approximately 150 employees nationwide with approximately
75 in the Colorado Marketplace. In.Attachment 1, you will find a listing of our key personnel
with a description of Training and Licenses held in various states across the country.
2. Clean Up Experience-
BELFOR Environmental performs a wide array of response cleanups ranging from the
small aircraft crash that occurred in Larimer County to asphalt Tanker rollovers in Poudre
Canyon in September 2009, or Diesel and Gasoline tankers rolling over in the mountain
passes of Colorado's high country.
Though these examples indicate BELFORS' competence in managing larger releases,
most responses are smaller and more routine. Each year we respond to numerous
Methamphetamine remediation and responses involving abandoned waste left in alleys and
parking lots. BELFOR has managed the City and County of Denver HazMat program since
2002 and the competitive bid for the contract was recently re -awarded to us, as well.

1. Payment is due in full within net terms stated on your invoice (typically net 30)
2. Prices are subject to change without notice.
3. All prices stated apply to Normal Turn Around Times.
4. Normal Turn Around Time (TAT) is 5-10 working days; faster TAT's will be
honored provided the laboratory workload will allow for rush samples. Our rush
premium's are as follows:
• Same Day Service- 4 times our published rates (Sample must be in
laboratory prior to 10am)
• 24 Hour service- 2 times our published rates
• 3 Day Service- 1.5 times our published rates
5. TLI will hold in confidence all information it receives from the customer and the
results of all tests and services provided to the customer.
6. Samples may be dropped off at the laboratory during normal working hours (M-F
8am-5pm) or shipped to the lab. TLI provides a courier service for the Denver
metro area & Northern Colorado at no additional charge (order minimum
required).
7. A finance charge of 1.5% per month will be added to any invoice past due.

3. Training and Safety -
Attached is an outline of our routine Safety Training for all Field employees. Additional
specialized training is also provided to Response Managers and Senior level managers at
the Transportation Training Center (TTCI) in Pueblo. TTCI is internationally recognized by
both private and public institutions seeking in-depth, comprehensive training solutions.
Tank Car Management, Blevy Management and Incident Commander are some of the
training courses available.

Though BELFOR Environmental does not have Industrial Hygienists on staff, we align with
a select group of Industrial Hygienists that can respond to our needs quickly and have the
experience in managing the typical contaminants of concern found throughout this region
and abroad: Methamphetamine, Mold, Asbestos, and Heavy Metal contamination are
some of the more common contaminants we manage with our Industrial Hygienist partners.

7. Standby Services -
As mentioned on page two, BELFOR maintains three rotating response teams. In the
instance that the first team is fully engaged on a response- whether standby or active, the
other teams can be called to duty. In the event that there is a large catastrophic
emergency, personnel and equipment resources will be mobilized from other BELFOR
offices.
In the case of very large and lengthy responses, BELFOR will hire additional resources,
screen all applicants through E-Verify and conduct orientation and applicable training
classes for those individuals in an abbreviated time frame, in order to respond effectively to
our client needs- without utilizing temporary services.

Due to the nature of our business, BELFOR Environmental does not hire subcontractors
to perform any of our core services. Limited subcontracting for ancillary services such
as Cranes, Electrical and paving are used- infrequently. More common "subcontracted"

10. Identification and HazCat procedures
Our process for "Field Identification" of unknown materials generally identifies chemicals
into hazard classes/characteristics, for example, flammability, corrosiveness, oxidizing
properties, and chlorine presence. 'This level of knowledge will enable the material to be
satisfactorily profiled into a TSDF or Non Hazardous waste disposal facility.
These tests are performed through usage of test kits, test strips, and instrumentation
and meters that BELFOR inventories internally.
11. Lab Pricing -
Laboratory services are generally charged at a cost plus 15% as outlined on Attachment
5 and 6. Attachment 11 is the Base Cost sheet from Technology Labs in Fort Collins, to
which the 15% is added.
12. Provisions for contract personnel during large scale clean-up projects -
In and effort to maintain the highest level of quality and security, BELFOR Environmental
performs all services with full time personnel. As part of this protocol, BELFOR performs
Alcohol and Drug testing on those individuals hired following an E-Verify process to
ensure all workers meet our rigorous employment standards. One recent example of
this was the hiring of 40 workers to assist on a flood- related asbestos abatement, and
blood borne pathogen decontamination project we were awarded in Memphis, TN in the
spring of 2010.

Regulatory Citation Summary
February 2010
2008 Regional Air Pollution Control Agency, Ohio
On February 26, 2008, RAPCA issued RMCATBelfor Environmental a notice of violation
for issues found during a December 4, 2006 inspection of our operations while performing
Emergency Asbestos Abatement activities in the Kettering Tower in Dayton, OH. The
violation involved inadequate wetting of materials during abatement of ACM fireproofing.
These issues were immediately corrected. The resulting civil violation involved a fine of
$19,000.
2010 U.S.- Department of Labor, Occupational Safety and Health Administration, Denver
Area Office
On February 4, 2010, OSHA issued BELFOR Environmental a Citation and Notification of
Penalty for work conducted August 5, 2009. This involves one Citation with one item, Type
Other, for improper fit test of three employees during abatement of thermal system insulation.
The proposed penalty is $0.00. This citation is under disputeiat this time.
2010 U.S. Department of Labor, Occupational Safety and Health Administration, Albany, AT
Office
On February 8, 2010, OSHA issued BELFOR Environmental a Citation and.Notification of
Penalty. This involves two Citations as follows.
Citation 1
Item 1, Serious: No End -of -service -life indicator in use for air purifying
respirators. Proposed Penalty $1,375
Item 2, Serious: One employee did not receive a medical evaluation to
determine ability to use respirator. Proposed Penalty $3,500

13. Ability to store hazardous, special and non -hazardous waste until final disposal -
Though we manage and transport Hazardous Materials on a daily basis and are suitably
qualified to do so, our facilities are not licensed for the storage of Hazardous Materials.
Historically, our clients have allowed for "Safe Haven" storage of waste materials at a
designated city location. In most cases, materials can be profiled and disposed of within
a day or two of submitting a profile.
14. Elements of a Closing Report -
A BELFOR Closure Report consists of:
Where did the release occur and to what was the extent of contamination.
Why, do we speculate that it happened?
Who the responsible party is, who is affected as a result.
What was the material released, apparent cause and result of the spill
What was the timeline associated with the response activities and subsequent cleanup
process.
That basic information is compiled in association with pictures Daily Work Reports,
analytical and Transportation and Disposal documentation quantifying the impact of the
release'that can be used for subsequent discussions or litigation.
15. Amount of Professional Liability or Errors and Omissions insurance.
These types of coverage fall under our General Liability Insurance policy which has a
$2,000,000 General Aggregate limit. See Attachment 15 for a copy of your Certificate of
Insurance.

Matt Wetzel, Senior Vice President of Operations
Mr. Wetzel has more than 18 years experience in the environmental industry. As senior vice president of
operations for BELFOR Environmental, he manages all aspects of the company with operations that provide
hazardous material emergency response and remediation services. Mr. Wetzel has managed more than 1,000
emergency responses, site investigations and remedial cleanups. These projects range from $1,000 responses
to $4 million cleanups for federal, state, and commercial clients. Projects involve a wide range of hazardous
materials including acids, chemical warfare agents, radionuclides, chlorinated organics, hydrocarbons, PCBs
and dioxin. Mr. Wetzel has prepared and implemented project work plans and budgets and supervised field
cleanup personnel in a wide variety of project tasks including facility decontamination and demolition,
installation and operation of waste water treatment facilities, excavation of sludge removal, contaminated soil
and dewatering, drum removal and waste transportation and disposal.
David Babbs, General Manager - Denver Asbestos
Mr. Babbs has over 15 years experience directing Health, Safety and Environmental Compliance in both
governmental and private industry remediation projects. He's been the Owner's representative and point -of -
contact in assessing, planning, managing and remediating facilities contaminated with asbestos, beryllium,
cadmium, radiation, mold, lead, sulfuric acid, mercury and arsenic. He also has over 15 years of hands-on
health and safety experience with DOE, DOD, EPA, FAA, Army, refineries, mining and schools. He conducted
Beryllium and Asbestos characterizations of DOE facilities prior to D&D. He has 15 years experience assessing,
directing and managing large complex Federal projects. He developed the Health and Safety Plan and enforced
its requirements at the EPA.Superfund site in Cushing, Oklahoma, Hudson Oil Refinery.
Chris Davis, General Manager - Atlanta Operations
Chris Davis is the General Manager for Asbestos Operations in the BELFOR Environmental Atlanta Regional
Office. He is integrally involved in all aspects of the asbestos industry. He started as a supervisor and developed
into project management, operations and general management since 1989. Mr. Davis' management experience
in the asbestos industry and as owner/operator of his own company is a key component to our success in
BELFOR Environmental's expansion into Atlanta and the Southeast Region of the United States.
Matt Hourigan, Special Programs Manager
Matt Hourigan has more than 11 years experience in the environmental industry. As Special Programs Manager,
he manages all national work and acts as liaison to BELFOR Property Restoration (North America's largest
restoration contracting company). Mr. Hourigan has responded to dozens of large scale catastrophic events
such as Hurricane Katrina and most recently the Iowa floods (2008) and Hurricane Ike (2008-2009). The range
of services provided under the national program include asbestos abatement, lead abatement, hazardous
materials spills and remediation, chemical lab packing, oil/fuel spill clean up, UST removal, confined space
entry, mold remediation and waste management. Mr. Houngan's career began with a BS in Environmental
Health and Science from Eastern Kentucky University and as Health and Safety Manager at RMCAT
Environmental. His career has progressed with positions including Government Services manager for a large
environmental company and Regional Asbestos Program Manager. Mr. Hourigan has managed over $100
million in the environmental remediation arena.

Mr. Johnston has more than 28 years of direct on -scene field experience with projects involving safe handling
and cleanup of hazardous materials at hazardous waste sites. He has directed and assisted over 2,000
emergency responses and remedial projects involving oil, chemicals and hazardous materials cleanup. He
managed ten projects under the ERGS Zone 4A contract and received a field performance rating of 70.4.
Through his extensive oil field experience, coupled with hazardous material site experience, Mr. Johnston gained
sound knowledge and experience for the safe handling of hazardous materials, radioactive waste site cleanup,
soil neutralization, waste characterization, sampling, lab packing, chemical transfer of rail cars and tank trucks,
chemical process equipment decontamination and removal heavy equipment operation, building
decontamination, building demolition, transportation and disposal of radioactive wastes, field construction, site
health and safety, work plan development and implementation, budgeting and cost controls.
Martin Keller, General Manager - Phoenix Operations
Martin Keller is an expert in the area of emergency response project management. He has been working in the
environmental services industry since 1993. His emergency response experience covers industrial,
municipalities, tribal and other service locations. He has incident commander training to compliment his project
management experience in waster management and remediation. His certifications include national EMT,
reactive chemical handling, compressed gas handling and railroad incident management. He is also a confined
space trainer.
Gregg Larson, General Manager - Portland Operations
Gregg Larson is an accomplished environmental professional with over 25 years of experience. His technical
expertise includes assessments, audits, emergency management, construction and remediation projects,
project management and training. Most recently coming from the private industry sector at Boeing, he has a
comprehensive background in disaster/emergency preparedness, compliance and many response programs.
Gregg has a BS from Oregon State University in chemistry and biology and is an Oregon certified hazardous
materials manager.
Sid Pacheco, Operations Manager - Medford Operations
Sid Pacheco came to the company with 12 years of experience as a project manager handling all levels of work
including estimating. In addition to this professional experience, he is a certified AHERA inspector. He has
certifications as a lead and mold inspector too. His work experience includes projects of all sizes from small
single day jobs to month -long projects such as decommissioning bakeries. He also has extensive experience
working with school district and within the Federal government arena.
