The City of Fort Collins has elected not to renew the policy with BCS Insurance Company for
transplant insurance coverage as a participant in the Western Cost Management Trust
(WCMT), per the above captioned proposal agreement, effective after December 31, 2010.
Coverage will be transferred to the City's employee medical benefit provider.
