The City of Fort Collins wishes to extend the agreement term for the above captioned proposal
per the existing terms and conditions and the following:
Due to the current economic climate, the City of Fort Collins will not be accepting any increase
in price; current contract pricing will be utilized for the year.
Any person (contractor) who operates a commercial motor vehicle, as defined in §382.107, in
intrastate or interstate commerce and is subject to the commercial driver's license requirement
of 49 CFR part 383 must be included in an alcohol and controlled substances testing program
under the Federal Highway Administration's rule. Documentation of proof must be
submitted with this renewal prior to performing work for the City of Fort Collins.
The term will be extended for one (1) additional year, September 16, 2010 through September
15, 2011. If the renewal is acceptable to your firm, please sign this letter in the space provided
include a current copy of insurance naming the City as an additional insured and return all
documents to the City of Fort Collins, Purchasing Division, P. O. Box 580, Fort Collins, CO
80522, within the next fifteen days.
If this extension is not agreeable with your firm, we ask that you send us a written notice stating
that you do not wish to renew the contract and state the reason for non -renewal.

All agreements and arrangements, written or unwritten, between and among employers and service
agents conceming the testing regulations implementation of DOT drug and alcohol testing requirements
are deemed, as a matterof law, to require compliance with all applicable provisions of Part 40 and
DOT agency drug and alcohol. Compliance with these provisions is a material term of all such
agreements and arrangements. All members must have a pre -employment drug screen done to become
a member of the Mobile Lab Highway Consortium.
The Department of Transportation requires employers to provide drug abuse and alcohol misuse
education. Mobile Lab will furnish a booklet called Rules and Testing lnfvrmation for Employees for
DOT requirements that you will be required to read. Mobile Lab will provide documentation that you
were given this required educational information.
All members of the consortium are subject to random drug and alcohol testing. Fifty percent of the
consortium will be drug tested on a random basis over the course of the year. Ten percent of the
consortium will be testers for alcohol. Each memberof the consortium has an equal chance of being
selected each time a selection is made. Thus some members may be randomly tested up to four times
in a single year while others may not be tested at all.
Tests are not included in the membership package price and must be paid for at the time of the test
unless other arrangements are made. This includes reasonable suspicion, return to duty, follow-up, pre-
employment, post accident, and random.
When notified that you have been randomly selected you must make arrangements to be tested that
day. No more than 2 hours can elapse before the end of your worts shift and the time of the test. Failure
to report to the agreed upon collection site at the specilied time is considered a "refusal to test'.. We
offer after hours and on site testing as an additional option because %Ae understand that it may be
difficult to have the testing done during work hours.
