LT SUMMARY OF WORK • Survey — Provided by City of Fort Collins — Engineering Department Survey Staff — 48 hour written request for survey • Testing — Provided by City of Fort Collins — Thru contract with CTL Thompson — Contractor will setup testing as required Traffic Control • Traffic Control — Responsibility of Contractor — Single lane closures in each direction of traffic — Review upcoming lane closure needs at each Weekly Progress Meeting Coordinate with CDOT with College Ave lane closures requires advanced City notification — Traffic control plans submitted to City 10 days before closure HIGHLIGHTS FROM GENERAL CONDITIONS AND GENERAL REQ'S: SUPERINTENDENTS: o The General Contractor SHALL employ a full time Superintendent whose sole responsibility is to monitor anff coordinate all facets of the work LIQUIDATED DAMAGES: o SUBSTANIAL COMPLETION: o The contractor will need to meet substantial completion requirements within 75 days of construction start date. Substantial completion shall be defined as the completion of all earthwork, rock work, concrete work, metal work, landscape planting material, Irrigation and electrical systems as required for this project. If the Contractor goes past the 75 calendar days for substantial completion, damages In the amount of One Thousand Eighty Six Dollars ($1,066.00) for each calendar day will be assessed. o FINAL ACCEPTANCE: Final acceptance will be met within 14 calendar days after substantial completion. Final completion is defined as all punchlist items and work that is defined in the contract. Liquidated Is in the amount of Five Hundred Dollars ($500.00) per calendar day will be charged if this schedule has not been met. Y HIGHLIGHTS FROM GENERAL CONDITIONS AND GENERAL REQ'S: • CONSTRUCTION HOURS: o Restricted Hours of Operation • 9:00 A.M. thru 3:30 P.M. - Monday thru Friday. ■ Given the scope of the proposed improvements, the City will permit extended working hours on a case by case basis — Extended Hours requests must be made in writing HIGHLIGHTS FROM GENERAL CONDITIONS AND GENERAL REQ'S: • PROJECT MEETINGS: o Onsite meeting in the field, time to be determined. o Schedules shall be updated weekly. The Contractor may be required to submit additional schedules depending on the progress of the project. • HAUL ROUTES: o Arterial and Collector streets only. Contractor will not be allowed to use residential streets to access the site with trucks hauling materials. HIGHLIGHTS FROM GENERAL CONDITIONS AND GENERAL REQ'S: • SITE CLEAN-UP: o The construction work shall be kept and accomplished in a neat, clean and orderly fashion. o Street sweeping may be required in order to minimize tracking and meet EPA storm water quality requirements. o Dust control will be required as necessary. Erosion Control - SWMP • Contractor will transfer the existing SWMP from Lafarge — NA for the portions of the worksite that are covered in the existing SWMP through the Colorado Dept of Public Health and Environment (CDPHE) • Erosion Control Devices are currently installed by Lafarge per the approved SWMP. • It will be the responsibility of the awarded contractor to maintain and repair the existing erosion control devices. • It will be the responsibility of the awarded contractor to do weekly SWMP inspections and turn original signature copies of the SWMP Inspection Report to the City. • This work is incidental to the cost of the project. b
