Project Description: Repair of three showers and upgrades to weather-stripping, door hardware and door
replacement at Fire Station 6 at 2511 Donnella Court
Scope of Services: Remove the and sheetrock from three showers to a point where no further damage is
present. Replace the backer board with a cement board and re -tile the showers. Repair and/or replace the
shower pans as needed on all showers. Retile shower pans as needed with 2x2 tiles and repair drains if
needed. Replace and repair weather strip on all exterior doors, add smoke seal where needed, replace
missing door closures and R&R one hollow core exterior door with an insulated door. Replace doors on
fuel shed with roll up door.

Service Provider agrees to perform the services identified above and on the attached forms in
accordance with the terms and conditions contained herein and in the Services Agreement between
the parties. In the event of a conflict between or ambiguity in the terms of the Services Agreement
and this work order (including the attached forms) the Services Agreement shall control.
