We built temporary trusses in order to tarp off some of the ' roofs. Out of 37 units, we had to board up all windows in 29 units as well as 17 doors. We enclosed the entire complex with a temporary fence to limit access to the property. We also ' provided security on the job site. After the buildings were secured, our mitigation technicians assisted the tenants in the safe removal of their property from the units. We had a total of 12 thirty -yard dumpsters on this job site for debris removal. We also had a skid steer and a front- end loader for debris removal. PDR provided temporary power, lighting and toilets on job site. Our Electrician and Plumber went through each unit disconnecting power, gas and water lines to ensure there would be no further damage. Mike Bates' home was severely hit; including damage to the roof and windows, several trusses were gone and the west wall was on the verge of collapsing. PDR, along with our Structural Engineer, started by shoring the wall, roof and ceiling with 2x6 braces to prevent them from collapsing. We boarded -up all windows and doors with OSB. We observed a hole on the west side of the roof where the wall was blown out and the trusses were missing; we secured that part of the roof and wall with tarps to prevent rain from coming in and doing more damage to the homeowner's content. The majority of the roof had been blown off — all remaining trusses were intact; this portion of the roof was secured with OSB. After the home was secured, PDR assisted Mr. Bates in retrieving immediate paperwork he needed for insurance claims along with valuables from the home. We inventoried and removed all of the home's content to our warehouse for cleaning and storage, pending insurance company review. A dumpster was placed on -site for debris removal. PDR was contracted by USAA and Mr. Bates to rebuild the home. This project was completed three weeks ahead of schedule and on budget. PDR braced the walls to prevent them from falling over. We then attached 2x4s to the trusses to prevent then from collapsing. The roof was severely ' damaged; several tarps were used to cover to prevent further damage. 2x4s were used to shore up the ceiling to prevent collapse. All windows were boarded up with OSB. Our electrician was called in to disconnect the panel due to damage on west wall and our plumber was called in to disconnect the gas line on the same wall. We inventoried and removed all the content to our warehouse for cleaning and storage for the insurance company. A dumpster was placed on site for debris removal. We were contracted by USAA and Mr. Morrison to rebuild the home. This project was completed on time and on budget. EMS Work Provided: PDR was called in by the Poudre Fire Dept. to provide an excavator and operator to assist the Fire Dept. in the demolition of a two-story home for safety reasons due to high winds. Fred Roberts operated the excavator with thumb attachment to remove debris and take down walls as directed by the fire dept. The OSB was cut to fit the frame of the window. 2x4's were then placed inside of the frame and holes were drilled through. The following list includes, but is not limited to, the types and various sizes of equipment available: skid ' steer, front-end loaders, backhoes, dozers, dragline, excavators, aerial equipment, forklifts and cranes and riggers — with cranes ranging from 28 ton to 500 ton. • Victim Assistance ' PDR has temporary housing service for immediate and long term housing options. This service includes items such as: emergency toiletries, emergency dry cleaning/laundry, ' emergency cleaning for preservation and valuables security. • Tree -Felling & Removal ' PDR has arrangements with our tree service companies for 24/7 emergency tree -felling and removal. Our tree service companies have all the needed equipment to handle all types of ' jobs. • Cleaning & Salvage PDR is capable of cleaning and restoring most any valuable items to the previous state. As ' with all restoration processes the results will vary case by case. • After Incident Assistance ' PDR can provide temporary offices, equipment storage, heated spaces, labor and information technology assistance to help restore communication and computer operations ' in order to reduce down time. Insurance company coordination. No request will be denied if the resource needed to complete the request can be located. Page 10 of 35 B: 1 Vendor Questions/Directives: 2: Describe your detailed Plan of Execution for PFA and City Board —Up Services: PAUL DAVIS RESTORATION & REMODELING PLAN OF EXECUTION PDR maintains an emergency board -up service plan in anticipation of service calls from the Poudre Fire Authority, City of Fort Collins and the Police Department. This plan defines emergency alert levels; establishes available manpower as well as the number of dispatches PDR is able to handle; describes organizational and personnel assignments and reporting procedures and emergency procedures; and includes a list of pre -arranged service providers and emergency contacts. Plan Purpose: Plan the efficient and safe board -up, shore -up and/or bracing of properties as necessary. These services will be provided within one hour of notification from the organization requesting the service. This plan will be a guideline that can be varied, as the situation requires. Plan Scope: This plan establishes: 1. 24 hours a day/7 days a week/52 weeks per year board -up service capability. 2. Confirmation of all phone calls within five minutes; arrival on scene — with qualified board - up staff — within sixty minutes; and offer to make any additional services required or needed available within a timely manner. 3. Completion of work to meet or surpass the standards of the requesting agency and ability to pass inspection and certification by the appropriate agency having oversight of the scene. 4. Securing of only those openings and/or areas as directed by the agency requesting service. 5. Reporting procedures that allow PDR the ability for reporting of conditions which could require additional services. 6. Detail listing of any/all material and/or equipment that is required for off -hour response, to include the ability to access sufficient security fencing for up to one acre perimeter. This plan will be reviewed periodically and continually updated by the PDR management and ownership. The official copy will be maintained in the PDR offices located at 309 Lincoln Court Fort Collins, CO 80524 and will be available to the contracting agencies at any time — producible immediately upon request. A critique of the plan and its effectiveness will be conducted following each board -up and emergency service response. The critique will generate improvements to the plan and formulate solution strategies with regard to noted weaknesses in the plan. A quarterly report Page 11 of 35 shall be issued to all the contracting agencies to inform them of the improvements and changes that are being implemented to this plan. Emergency Alert Levels and Outline of Manpower: Pre -Storm Watch This situation is prior to the arrival of anticipated storms or possible natural disasters. This is a precautionary situation that will follow a weather broadcast of a severe nature. The on -call Crew Lead will monitor the situation and advise the on -call Operations Manager of the possible threats. If deemed necessary, the on -call Crew Lead and/or Operations Manager will put the on -call Crew on high alert. Alert levels are defined as follows: Level 1 An emergency/after hours call for board up services where on -call crews are able to arrive at site requiring service in less than 1 hour without the assistance of outside crews. Personnel assemble as needed at PDR offices and start the emergency/after hour's protocol. Level 2 Multiple emergency/after hours call requests where reserve crews are able to arrive at sites requiring service in less than 1-hour. All construction, operations and service personnel shall be put on alert for the possibility of additional service calls. Level 3 An emergency/after hours request for resources where one of PDR pre -arranged service providers shall be required to fulfill the service request. The service provider will respond and arrive at the site requiring service within 2-hours. Level 4 Emergency/after hours requests in the event of a disaster were multiple dispatches are required. In this situation, all employees in both the Fort Collins and Denver offices will be notified and all personnel will be required to report to the office; where they will receive their assignments, report to the site and proceed with the required service. Available Manpower and Dispatch Capabilities PDR has over 45 employees and 20 vehicles with the capabilities to dispatch up to seven emergency response crews out of the Fort Collins location at any one time. Additionally, PDR has seven emergency response crews at the Denver location that are able to respond as needed. PDR can also handle any additional overflow of calls with dedicated on -call personnel. All employees at PDR are provided with company cell phones and are required to notify their immediate Manager of any personal "after hours" commitments. This reporting procedure allows the on -call Manager to immediately know who is available should the request for additional services be needed. PDR's contracted services have priority to basic calls for service. Page 12 of 35 a PDR makes staff adjustments during usually high volume periods (i.e. after freezing weather, floods or catastrophic events). Because our staff is cross -trained, we are able to provide vast and far reaching combined responses. If there are multiple calls over a short span of time, aPDR's backup system allows us to service those calls immediately for the requesting agency. IOrganization and Personnel Assignments/Responsibilities: ' On -Call Crew Job Responsibilities Relating to Emergency Service Calls Coordination and implementation of board -up service and any additional services required by the requesting agency during the entire period of any and all emergencies: ' • Check and inventory all board up equipment and materials to ensure that all required items for a successful response to the service call are available and operational. ' • Determine and/or anticipate problems and a course of action to follow; communicate all to the Project Managers or Owners. • Receive directions for priority of board -up service and securing the scene from the Project Managers or Owners. • Provide communications and status updates to the On -Call Crew Leader, the Project Managers and/or Owners. 1 • After board up service is complete and scene has been secured, patrol the scene to ensure all required services have been completed and all material and equipment has been accounted for and stowed properly in emergency service vehicle. • On -call crew is to meet on site and review the Service Call Checklist prior to returning to PDR offices. ' • Restock emergency service staging area, recharge all batteries, refuel all equipment and gather with On -Call Crew Leader, Project Manager and/or Owners for Service Call Debriefing. Normally, the emergency board -up takes place before the insurance company is notified. On ' the day of the loss, after the board -up is complete and the home or business is secured, we try to get as much information as possible from the homeowner or business owner; at minimum ' any applicable insurance company information and a signed Emergency Work Authorization, if possible. This only gives us authority to do the EMS; we then contact the insurance company ' and inform them of the loss and the services preformed. We have strict guidelines — mandated by each carrier — which we must follow on each loss. The ' primary, immediate objective from any carrier is to secure the property to prevent any further damage. Board -Up Procedures: • All openings in the basement, first floor doors and windows and any point of entry accessible from a porch, fire escape or other potential climbing point shall be barricaded with %" OSB/CDX plywood, 2x4 braces, carriage bolt sets and screws. Particle board, wafer board, Masonite or other similar material shall not be used for purposes of boarding -up a building. • Openings that are at least 10' from ground level, which are not accessible from a porch, fire escape, roof, or other climbing point, can be secured with screws in each brace and every 12" around the perimeter. For all openings, the plywood should be fitted so that it rests snugly against the exterior frame; butting up to the siding on wood frame buildings and up to the brick molding edge on brick buildings. It may be necessary to remove the staff bead so this fit can be flush and tight. • The structure shall be posted with a "NO TRESPASSING" sign at the completion of the board -up. Exit is made over a ladder when the last window is boarded. • Plywood shall be cut to fit over the window and door openings, flush with outside of the molding/trimmer stud. Application of barriers shall be completed so that all lift or pry points are avoided. Page 28 of 35 • The 2x4 braces shall be cut to fit the horizontal dimension of the plywood. Two exterior and two interior 2x4 braces shall be provided for each window and three sets for each door. • Window Assembly — Braces are located horizontally approximately 1/3 of the distance from the top and the bottom of the window. Bolt holes are located 1/3 of the length of the brace from the outside edge of the window jams. Prior to installation, the assembly should be pre -assembled and 3/8" holes drilled through all of the components. • Door Assembly — Door braces will be placed horizontally; one in the center of the doorway and one 1/2 the distance from the center to the top and one 1/2 distance from the center to the bottom of the doorway. Bolt holes are located 1/3 of the length of the brace from the outside edge of the door frame. Prior to installation, the assembly should be pre -assembled and 3/8" holes drilled through all of the components. • Plywood used to cover exterior openings shall be nailed every 12" along the perimeter to the window or door frame. • The 2x4 braces on the interior and exterior of the assemblies shall be secured using 3/8" by 12" carriage bolt assemblies. Bolts shall be inserted through the predrilled holes from the exterior with a 1/2" washer place against the exterior brace, a 3/8" washer is placed against the interior brace. The bolt is tightened from the inside so that it slightly compresses the interior brace. • The exterior surfaces of barriers shall be painted or stained the same color as the structure to minimize the appearance. The franchise was opened by Phil Majerus and was primarily doing water mitigation for direct repair programs with insurance carriers. Over the years our company has expanded to include a full service mitigation division, emergency services division, structure rebuild division, roofing division and a remodeling division. We have also expanded our territories to include all of Larimer, Weld, Laramie and Albany counties. ' Although the company has grown, our goal has remained the same: to provide great quality service to our customers 24 hours a day, seven days a week. Our mission statement is, simply, ' "We Restore Lives". Mark Hughes and Fred Roberts purchased PDR in February of 2007. Mark Hughes has ' over 30 years' carpentry experience and Fred Roberts has over 10 years in the restoration industry with 20 years of management experience. Since the purchase in 2007, PDR has been focused on expanding our customer service relationships; from homeowners, to our national program relationships with insurance carriers. One of the ways we have accomplished our excellent customer service reputation is by ensuring our clients that a trained PDR&R employee ' will answer the phone 24/7. We also established a process in which and independent company surveys each and every customer at the end of all jobs to see how we did - whether good or bad. Customer satisfaction survey results are then provided to our PDR business and PDR corporate in order to make sure we are upholding the highest standards possible. A Project Manager is assigned to each job from start to finish; helping the customer with questions on the process of putting their home back together and acting as a liaison with the insurance adjusters. ' Description: ' On May 22 2008 we responded to Windsor CO. after a tornado went through the town devastating the whole community and doing several million dollars in damages. USAA, Kemper, Travelers, Farmers, Met Life and several other insurance companies called us in to do EMS shoring and board -ups for their insured. We boarded up a total of 32 homes, along with an ' apartment complex consenting of 7 buildings and a total of 37 units. We had a total of 11 Carpenters, 14 Laborers, 5 Mitigation Technicians and 6 Project Managers. We installed % mile of temporary fencing along with security gates. We aided tenants and homeowners in the safe ' removal of their property from their homes and apartments and provided 6 pods for storage and a total of 39 thirty -yard dumpsters. We supplied temporary lighting and toilets for the ' apartment complex. All work listed above, along with multiple others, was completed in 7 days. The following three board -ups are just a few of the jobs that were done in Windsor. Fort Collins, CO 80526 PH: 970-282-7522 Cell: 970-631-2818 Chadrick Martinez called in PDR on May 22 2008 after the Cottonwood Apartments complex was hit by the tornado in Windsor. When we arrived, I met with Chadrick to evaluate the damage. The complex consisted of 7 buildings - all of which were damaged. Of the 7 buildings, 5 had extensive damage with major roof and truss damage. One unit had a wall that had partially collapsed on the second story; the entire roof was gone, including most of the trusses. We immediately had a Structural Engineer come in and evaluate the damage and assist us in coming up with a safe plan to brace the walls and shore -up the floors, ceilings and trusses.
