Project Description: Complete renovation specific inspection for asbestos -containing materials, lead -
based paint screen, and hazardous materials inventory at City of Fort Collins — Lincoln Center located
at 417 West Magnolia Street, Fort Collins, Colorado 80521
Scope of Services: Collect samples of all accessible suspect asbestos -containing building materials
and submit samples to laboratory for PLM analysis to determine asbestos content_ Perform
confirmatory point count analysis on materials containing less than or equal to 1 percent asbestos.
The lead screening will include collecting a sufficient number of readings using a field portable X-ray
Fluorescence (XRF) analyzer to test painted building components for the presence or absence of lead -
based paint. The hazardous materials inventory will includes conducting a renovation specific
inspection and compilation of an inventory of presumed common "hazardous building materials" where
proper disposition of these materials is customary prior to renovation/demolition to comply with
applicable regulations and general industry standards. All inspection, sample collection, and lab
analyses activities will be performed in accordance with customary (industry standard) practices. Both
interior and exterior portions of the structure scheduled for renovation will be inspected.

contained herein and in the Professional
Services Agreement between the parties. In
the event of a conflict between or ambiguity in
the terms of the Professional Services
Agreement and this work order (including the
attached forms) the Professional Services
Agreement shall control.
