Experience On a separate sheet, provide concession operating experience for yourself and your staff. 2. Financial On a separate sheet, please provide information on your financial ability to operate this concession. 3. Products On a separate sheet, list the products you intend to serve along with proposed prices. 4. Other Information Have you ever held or do you currently hold any financial interest, direct or indirect, in any retail Jigger establishment, distillery, importing or wholesale liquor license? YES _ NO If "YES", explain such interest on a separate sheet. Attach copies of any written agreement or details of any oral agreement, by which any person (including a corporation) will share in the profit or gross proceeds of this concession, and any agreement relating to the concession which is contingent or conditional, in any way, upon volume, profit, sales, giving of advice or consultation: Do you plan to be an active participant in the management and operation of the concession: YES �'-NO _ If "NO", on a separate sheet, explain your intended management arrangement. 5. P11.27 Qualification Statement Form Experience- Have been the concessionaire at The Fort Collins Senior Center since 2002. Was previous owner of the concessionaire at Collindale Golf Course. 2. Financial- I have proven my financial ability as I have been the concessionaire for the last 6+ years. I have no financial issues as I am well able to capitalize insurance and have already paid 2008 liquor fees and taxes, I have a full working inventory and NO expected outlays other than business as usual in the near future. I Please find a price sheet attached with proposed prices. Other information: Please see Exhibit A for filled out portion. I currently hold the liquor license for the Fort Collins Senior Center. Any outside alcohol or beverages will be removed. • Bottom's Up Bar Service will determine the number of bartenders required at each event. • A minimum of $150.00 in sales is required. • Plastic -ware is used unless glassware is requested. Glasses rent @$.45 each -- 2 glasses per guest. • Legal age to consume alcohol is 21. Bartenders are required to request a picture ID, and retain the right to refuse service to anyone for any reason. • An 18% Service Charge will be applied to all special orders and hosted bars. Hosts are responsible for the actions of their guests during function time. • State Liquor Laws state that no liquor (whole or part) may leave event premises with the exception of wine bought by the bottle, corked and with a receipt from bartender.
