independently Contracted Quality Control inspections
Parties: City of !sort Collins, Colorado Mosquito (:ontroi, Inc.
The purpose of this letter is to set forth an agreement between all parries so chat an
understanding of the goals, methods and responsibilities of ea& party are clear and the
program can proceed in an efficient and effective manner. As CMC understands it, the
goal of the Quality Control inspections is to provide data to the City of Fort Collins
which would measure the efficacy of larval and adult mosquito control product selection
and effectiveness of the larval and adult mosquito control operations In the City of Fort
Collins mosquito control program. This data will then be used in future planning and
decision making processes to make appropriate improvements :;n the City's continuing
efforts to control mosquitoes and resultant mosquito -borne diseases.
CMC agrees to work cooperatively with the City of Fort Collins and its quality control
employee by providing larval site inspection data and allowing ancess into CMC's office
location for that purpose. CMC understands and agrees that quality control inspections
by the City are aimed at 1) identifying the need for additional :uroas to be added to the
inspection and application program presently in place 2) to investigate potential
unidentified/uninspected sites within the present boundaries, which have not changed
since the 2004 implementation of the program 3) ensure that the labor/staffing hours
invested into the larval inspection program are sufficient, t) larvicide application
treatments are achieving the goals of reduction in larvae throagh appropriate product
selection, application rate and application timing (for larval stage) of products applied to
control larval mosquitoes and (5) larvicide application treatments are reducing the
necessity fbr adult mosquito control applications.
All partial understand and agree that . information obtained, f3'om CMC's records
regarding 1) assessment 2) findings of inspections 3) eviluation of application records
and 4) all larval data site locations are to be provided to the City of Fort Collins for
internal use only and will be considered confidential;,
The City of Fort Collins agrees to:
1. Develop quality control site inspection lists, directions and work maps
independently, with little or no direct input or inquirer of CMC personnel or
management
2. Access to inspection and larvicide (ML) application data records is available
directly from the CMC Daily i&L shoots Found in CMC route books. CMC does
not have the capacity to supply a computer, printer or work station for the City to
obtain electronic information directly. Any requests for Aectrome data or records
must be in writing, Data for written requests will be supplied to the City as
quickly as possible, within the week of request.
3 Larval site maps and route inspection books are not to be removed from Colorado
Mosquito Control's office location.

4. Review of data records and route books used in CMC's amtracted operations may
be performed only during normal business hours (7--VAM-5:00PM Monday
through Friday). It is recommended that the City notify C.MC prior to arriving at
the office location, Notification will reduce wasted time by the independent
contracted in the event business doors are locked or all staff members are in the
field at time of dosired review. The City will not be provided a key to the CMC
office.
5 The City will direct all questions to the CMC Operations Manager only, These
communications will be made via phone calls during business hours, direct
communications in the CMC office to the manager, or via email. A report of
findings will be available for CMC review. This will enable CMC to address any
concerns or problems found by the City. A daily e-mail will be sent to the CMC
Manager with immediate issues and a weekly report will sum up the findings for
the week.

control program only. Data concerning other CMC itecounts, site maps, and
operational intbtmation will not be requested for accowds other than the City of
Fort Collins.
7 The City employee will not be allowed to operate any equipment, machinery,
tools or ride in any CMC owned vehicles
8. Nothing in this memo changes any of the terms or provisions of the City's existing
services agreement
