Re: Sloans Lake Managed Care - Renewal
Dear Vince:
We have been advised, in writing, by Sloans Lake Managed Care (SLMC) that they are requesting a
4.3% access and UR fee increase effective January 1, 2000. SLMC is also requesting that in order to
assist in the directing of claims to the appropriate payor, that you use their internal SLMC
identification number on all ID cards. Your SLMC identification number is 316. SLMC will assist
you in this process by supplying new camera artwork with the SLMC # already on the logo. The
current and renewal rates are shown below:
