DESCRIPTION: The construction of a sand/salt storage dome, a control building and liquid dispensing
equipment, complete with related site work and landscaping.
Reason for change: Incorporation of various Additional Work Authorizations.
2. Description of change: WO-00001 — Change the flooring in the Control Building from VCT to ceramic
tile - $5,145.00; WO-00002 — Delete the fire sprinkler system in the Dome Building — ($66,584.00); WO-00003
— Delete over -excavation from the scope of the earth work — ($32,325.00); WO-00004 — Changes in the
foundation design (haunches) that required additional rebar and placement - $1,196.00; WO-00005 — Change
overhead doors sizes to 16' X 20', including extra cost for door finish - $2,091.00; WO-00006 — Deduct all
sealant scope of work from the exterior of the Dome Building, substitute Lapidolith for the Euclid Surfhard, add
Hydozo Silane 40 VOC to interior of Dome Building and add sealer for 10caps at the exterior of the Dome
Building - $6,769.00; WO-00007 - Credit for electrical service downsizing, deletion of 2" communication
conduit, inverter downsizing, deletion of communication conduit required by note #10, deletion of the fire alarm
system and deletion of the semi -automatic feature on the transfer switch and add for the off-load pump
connections, conveyor controls, spray bar pumps and controls and motor connections - $12,321.00; WO-
00008 — Add landscaping around the Control Building - $3,797.00; WO-0009 — Changes to mechanical per
Addendum #3, including pumps, valves, piping and equipment - $39,772.00; WO-00010 — Supply and install
remote mounting kit, signal converter junction box, mounting brackets and 300 feet of cable to each meter -
$2,067.00.
3. Change in Contract Cost: ($25,751.00)
4. Change in Contract Time: 115 Days — Substantial Completion is now September 18, 2006.
