Dear Ms. Halpern:
This letter is in acknowledgement of the increase in the charge per employee per month rate
from $3.79 to $3.54 for renewal of the above referenced agreement for Group Dental Plan #'s
1857 and 1858.
The term will be extended for one (1) additional year from January 1, 2006 through December
31, 2006. Rates are effective through December 31, 2007.
