1. Reason for change. 2. Description of change.
3. Change in contract cost. 4. Change in contract time.
1.&2. Due to direction from City Council (2128/06), the contract completion date will require another extension.
Requested tasks such as crafting alternative ballot language for Council consideration are within the scope of work
so there will be no increase in contract amount. Expected completion date for this project is now September 30, 2006.
3. There will be no change to the contract cost.
4. The contract time will again be extended from a completion date of March 31, 2006 to September 30, 2006.
