and subject to all condit' ns thereof, the undersigned
Don Kehn Construction, Inc. a ** Corporatio Limited Liability Company,

in the State of Colorado hereby proposes to furnish and do everything
required by the Contract Documents to which this refers for the
construction of all items listed on the following Bid Schedule or Bid
Schedules.
2. The undersigned Bidder does hereby declare and stipulate that this proposal
is made in good faith, without collusion or connection with any other
person or persons Bidding for the same Work, and that it is made in
pursuance of and subject to all the terms and conditions of the Invitation
to Bid and Instructions to Bidders, the Agreement, the detailed
Specifications, and the Drawings pertaining to the Work to be done, all of
which have been examined by the undersigned.
3. Accompanying this Bid is a certified or cashier's check or standard Bid
bond in the sum of Five Percent ($
5% ) in accordance with the Invitation To Bid and
Instructions to Bidders.
4. The undersigned Bidder agrees to execute the Agreement and a Performance
Bond and a Payment Bond for the amount of the total of this Bid within
fifteen (15) calendar days from the date when the written notice of the
award of the contract is delivered to him at the address given on this Bid.

proposes to furnish the specified performance and payment bonds is as

6. Payment for Work performed will be in accordance with the Bid Schedule or
Bid Schedules subject to changes as provided in the Contract Documents.

Description of Bid #5834 Ziegler Road Widening Project
To all prospective bidders under the specifications and contract documents described above,
the following changes are hereby made.
1. In the Technical Specifications SECTION 609 CURB AND GUTTER the following information
was omitted but should be included and shall read as follows:
Under section 609.01 the following should be included:
Concrete Bike Lane/Curb and Gutter— This work shall consist of the forming, poring and
saw cutting of joints for a monolithic concrete combination Bike Lane/Curb and Gutter from
stations 25+23.66 to 28+43.85 as shown on page 16 of the Drake/Ziegler Improvement plans.
All of the materials, labor and equipment necessary to complete this work shall not be paid for
separately under any other item in these plans and shall be considered incidental to the work.
This item shall be paid for by the lineal foot — (LF).
Also, under section 609.07 the following shall be added
609-07 Concrete Bike Lane/Curb and Gutter — (LF)
II. Included in this addendum is the pedestrian access ramps with truncated domes detail
drawing.
111. This Addendum also includes a Corrected Bid sheet with the added bid item from this
Addendum.

Description of Bid #5834 Ziegler Road Widening Project
PRE -BID DATE: January 27, 2004, 1:00p.m. Conference Rooms A and B, at 281 N. College
Ave., Fort Collins.
To all prospective bidders under the specifications and contract documents described above,
the following changes are hereby made.
1. Section 01330 Survey Data in the General Requirements should read as follows:
SECTION 01330
SURVEY DATA
1.1 SURVEY REQUIREMENTS
A. The Owner will not provide the construction surveying for the Project. The Contractor will
be responsible to provide the surveying required on this project.
B. The Owner will make the projects CAD drawings available to the Contractor upon request.
C. The Contractor shall protect all survey monuments and construction stakes. If it is
unavoidable to remove a survey monument or construction stakes, the Contractor is
responsible for notifying the Surveyor and allowing enough time for the monuments or
stakes to be relocated. The Contractor will be responsible for the cost of restaking
construction stakes and for the cost of re-establishing a destroyed monument.
END OF SECTION
II. In our General Requirements for this Project under SECTION 01510 TEMPORARY UTILITIES
under section 1.1 paragraph B should read as follows:
B. Contractor will furnish water in reasonable amounts for execution of the work at existing fire
hydrants without charge to the Owner. The Contractor shall Coordinate and schedule with the
Owners' Field Representative for the Water Department to select the appropriate fire hydrant
and to set the hydrant meter.

III. In the Technical Specifications SECTION 607 FENCES was omitted but should be included
and shall read as follows:

FENCES
Section 607 of the Standard Specifications is hereby revised as follows:
Subsection 607.01 is revised to include the following:
4 Strand Smooth Wire Fence — This work shall consist of all of the materials, equipment and labor
necessary to install 4 strand smooth wire fencing. All the materials including posts and the work
necessary to set them shall be considered incidental to the work and not be paid for separately This
item shall be field measured and paid for by the lineal foot.
Subsection 607.04 is revised to include the following:
The accepted quantities will be paid for at the contract unit price.

The foregoing prices shall include all labor, materials, transportation,
shoring, removal, dewatering, overhead, profit, insurance, etc., to cover
the complete Work in place of the several kinds called for.
Bidder acknowledges that the OWNER has the right to delete items in the Bid
or change quantities at his sole discretion without affecting the Agreement
or prices of any item so long as the deletion or change does not exceed
twenty-five percent (25%) of the total Agreement Price.
