COMMUNITY DEVELOPMENT BLOCK GRANT FUNDING
THIS AGREEMENT is made and entered into this 18th day of January, 2005,
by and between THE CITY OF FORT COLLINS, COLORADO, a Colorado municipal
corporation (hereinafter referred to as "the City), and CATHOLIC CHARITIES
NORTHERN [CCN] (hereinafter referred to as "the Subrecipient),
WITNESSETH:
WHEREAS, the parties hereto previously entered into a Subrecipient Service
Agreement for Community Development Block Grant (CDBG) Public Service funding
dated September 23, 2004 (the "Contract") which provided for the rendition of certain
services by the Subrecipient and a distribution by the City to the Subrecipient of certain
funds (TWENTY FIVE THOUSAND DOLLARS - $25,000) made available to the City from
the United States Government under Title I of the Housing and Community
Development Act of 1974; and
WHEREAS, certain text inaccuracies have been noted in the contract; and
WHEREAS, changes within the Subrecipient's organizational structure necessitate
a new address for communications, payments, and notices; and
WHEREAS, the parties hereto are desirous of modifying the contract with respect
these items;
NOW, THEREFORE, in consideration of the mutual promises of the parties and
other good and valuable consideration, the receipt and adequacy of which is hereby
acknowledged, the parties agree as follows:
1. ARTICLE 1. PROJECT, Section 1. Scope of Services, B., Subrecipient Shall" (Page
1, last paragraph), is amended as follows, to reflect the total awarded amount
($25,000) as opposed to the total requested amount ($30,000) in the
Subrecipient's Spring 2004 Competitive Process application:
"The CDBG grant funds of $11,250 $13399 will be used to assist with .54 -6S
FTE for the salaries of several Hostel of Hospitality employees (usually 3-7),
working part-time (normal range of 12-36 hours/week), and earning an average
rate of approximately $8.50/hr. CDBG grant monies of $13,750 $16399 will
also be used for .42 39 FTE of the Supervisor's salary."
2. The total awarded amount of $25,000 is correctly noted under Section 2., Project
Description, Funded Amount, on Page 2 of the Contract.
3. ARTICLE II. FINANCIAL CONDITIONS, SECTION 1. BUDGET AND
COMPENSATION, Budget Summary:, Where Payments Are Made:, Page 3 of
the Contract, is amended as follows:
