SCOPE OF WORK — LOMR FOR THE PEDESTRIAN BRIDGE AND BIKE TRAIL EXTENSION ON THE CACHE LA POUDRE RIVER PROJECT OBJECTIVE: To submit a LOMR request for the Pedestrian and Bike Trail Bridge over the Cache La Poudre. GENERAL The Consultant will provide services as required to complete a post construction Letter of Map Revision (LOMR) for the Pedestrian and Bike Bridge over the Cache La Poudre River. Ayres Associates designed the bridge and completed the initial CLOMR application, which was approved by FEMA. The LOMR will be submitted to document as -built construction and the project floodplain for FEMA's approval. The bridge structure was completed in May of 2004. This task will include updating the hydraulic models with the as -built construction data and preparing a LOMR application. TASK 1 — COORDINATION AND MEETINGS 1.1 Meetings and Correspondence. An initial meeting was held at the Ayres Associates Office between the Owner, consultant and project engineers. This meeting and the following discussions are intended to determine the background information for the project and what resources are needed. Up to 3 additional meetings are included and will be held as needed, with City staff. In addition, correspondence and up to 2 meetings will occur after submittal, to respond to comments, TASK 2 — DATA INSERTION AND MODELING 2.1 Update Hydraulic Models With As-sulit Information. The as -built Information will be incorporated into the various hydraulic models. Although some.changes to the model geometry may be needed, it is assumed that the new as -built information will not be extensive. 2.2 Re -run All Hydraulic Models. The various hydraulic models shall be re -run to reflect the post construction data and to ensure that no changes to water surface elevations at the bridge have occurred due to changes in the construction phase of the project. TASK 3 -- DATA EXTRACTION AND REPORTING 3.1 Insert Modeling Output Into Report Figures, Tables, Text. After the models have been updated and re -run, output data must be extracted and disseminated. This data must then be inserted into the appropriate text, figures, and tables. 3.2 Update Text of Report for Built Status. Since the CLOMR submittal report was written before construction, it will need to be updated with the new information that confirms that the CLOMR requirements detailed in the report were met. 3.3 Update Appendix Information. The technical appendix will be updated with the new model cross -sections and input. In addition, the revised model output will be inserted into the appendix. 02/28/2005 MON 10:04 FAX 970 221 6586 CITY OF FT COLLINS CLRS 1000/004 3.4 Prepare MT-2 Form. The MT-2 form that was prepared for the CLOMR application will be updated and submitted far the LOMR application. 3.5 Prepare Report Document for Submittal. The last step to complete the LOMR submittal package will be the production of 5 reports and the associated data CD's, including reproduction of all color maps and figures, 3.6 Submittal Fee. TASK 4 — QA/QC 4.1 Review and Redllning. As part of every submittal, a quality control review will be completed to ensure that the LOMR requirements are met.
