We are in receipt of the signed renewal letter for the above referenced project indicating
your acceptance of revisions A through D inclusive. Per your request the following
revisions "E" and "F" will also be effective for the term January 1, 2005 through
December 31, 2005:

Re: Renewal, P918 Paratransit Services
Dear Mr. Hofman:
The City of Fort Collins wishes to extend the agreement term for the above captioned proposal per the
Agreement signed February 26, 2004. The term will be extended from January 1, 2005 through December 31,
2005 per the existing terms and conditions with the following revisions:
A. 6. Contract Sum.
REPLACE 6. Contract Sum. with: The City shall pay the Service provider for the performance of this
Contract, subject to additions and deletions provided herein, the sum of $15.45 per trip.
DELETE: Additional trips within the % mile area will be billed at a rate of $12.00 per trip.
B. Attachment C
ADD: City of Fort Collins Substance Abuse Policy: Service Provider must comply with current
substance abuse policy and any changes to the policy as made. The City is responsible for
providing the Service Provider with the current policy.

wish to renew the contract and state the reason for non -renewal.

8. Maximum riding time on vehicles:
The maximum time any rider shallwill be required to spend on board avehicle sha►Iwill
be one (1) hour. The Service Provider will not schedule manifests that do not
meet this standard and the GsatrasterService Provider shallwiil adhere to this standard
during its operations. If ridership demands exceed the Gea#aetefService Providers
capabilities in scheduling to maintain this standard, the CORtFAAWrService Provider will
immediately notify Dial -A -Ride.
PERSONNEL REQUIREMENTS:
Driver Pre -hire Qualifications

at fault for any incident that would prohibit that driver from maintaining a Commercial
Driver's License, whether or not the driver holds one, the driver is no longer eligible to
perform duties under this contract. Failure to immediately remove an ineligible driver

copies of the report.

Other misdemeanor criminal convictions that would disqualify an employee include, but
are not limited to:
• Any offense that requires registration of a sex offender
• Any driving under the influence of drugs or alcohol violation within the past 3
years
• Any offense of violence by a person in a position of trust.

➢ DRUG TESTING: All safety -sensitive applicants are required to abide by the City of Fort
Collins Transit Substance Abuse Policies and Procedures, which includes submitting to
a pre -employment drug test. Employees will be tested for those controlled substances
specified in the Procedures for Transportation Workplace Drug Testing Program (49
CFR Part 40).
