PROPOSAL #P966
AOSafety as part of the Aearo Company as registered with the Purchasing
Department of the City of Fort Collins, presents the following submittal for
proposal for #P966.

AOSafety is also the current provider for this program for the City of
Fort Collins.

d. AOSafety will provide the City of Fort Collins with a program that
will allow employees to purchase Safety Eyewear through the AO
Dispensing Vendor as required and guidelined through the City of
Fort Collins Safety Office. All products are ANSI Z87.1-2003
updated as required in August 2003, and designates AOSafety as a
High Impact Lab/Mfg of Safety Eyewear. The 2 program pricing
options are included and provide the employee vast options while
providing the City competitive pricing guidelines.

Please see the attached Dispensing panel list. These providers have
established and in business for over 5 years in Fort Collins.

Frames are provided to the panel by AOSafety and will be updated by the
AO ASM at least 3 times per year. All invoicing is individual style and
itemized to add dispensing fees. The City of Fort Collins will ONLY
receive an invoice from AOSafety and not the dispensine vendors.

High impact protection:
Polycarbonate is the most impact resistant lens material available in the market and is the only lens
material that meets the ANSI Z87.1-2003 Standard for High Impact Protection and as such will be
designated with a plus (+) sign on the lens. Glass and Plastic lenses meet the ANSI Z87.1-2003 Standard
for Basic Impact Protection, but do not meet the regov'ements for High Impact Protection.

WARRANTY
AOSafety will replace, at no charge, all prescription eyewear products if the
following conditions occur within one year from date ofpurchase:
a) Lens breakage (not a warranty against scratches)
b) Frame breakage
c) Coating failure (peeling or crazing)
d) Corrosion of any frame compment
Warranties apply provided that no abnormal in excessive use or abuse is
evident. Product must be returned to AOSafety to allow, a thorough
inspectim of the defective merchandise.
If a replacement part is not available, AOSaSoty will provide a new frame or
am complete pair of eyewear at the repair part price.
Extended Warranty: All Group F, G and G PLUS frames are guaranteed
for 2 years subject to the conditms above.
A limited one-year scratch warranty is offered on SuperCote. This warranty
includes work eavimmnents where lens scratching is at high risk, including
but not limited to, mining, griudmg, and welding. All claims fur watch
warranty must be returned to AOSafety for evaluation before credit can be
issued Limited to one replacement per order.

POLICIES AND PROCEDURES
Fitting and Dispensing
Carefid consideration should be given to comfort and fit It is AOSafety's
intention that all prescription eymcar be verified and properly fitted by an
eye care professional. Continued wearing of eyewear is more likely when the
eyewear fits properly. Prescription safety frames are generally available in a
variety of sizes and care should be taken to ensure that the proper size is
selected
Prescription lens rephncement
Unless prohibited by state law, AOSafety will process an order for eyewear
where the prescription is less than two years old We urge our customers to
visit their eye care professional to ensure that their prescription is accurate
and that their eyes are healthy.

AOSafety recommends Polymbonate or Plastic lenses in fiames with eye
sizes of 54 mm or greater.
Fabrication Errors
AOSafety optical laboratories were the fast to be ISO certified in North
America. Quality is our first priority, but should we make an eror, we will
gladly remake the prescription eyeweer at no charge
Full Line Multifecals (Executives)
Processing full line multifmals (also called Executives) in Glass ever 52
mm, or Polycarbonate or Plastic over 54 men, may adversely affect the
optical properties and visual performance of the lessee. AOSafity
recommends that you consult your eye care professional berate ordering full

TERMS AND CONDITIONS
Payment Terms: Net 30 days • Prices subject to change without notice.
Any request for changes to a prescription due to doctor's or optician's
changy will be processed according to the following policy:
1) AOSafety will replace any prescription or order change at no charge if
the change is equal to or Im than the original. A request for a more
wepeosve replacement will require a charge for the difference in price.
Subsequent prescription or order changes for the same customer will
be charged at 100%.
2) AOSafety will charge 50% on any prescription or optician change
request after 60 days from purchase.
3) Any request after stir months from purchase date will be subject to an
invoice for full price.
Any request for an order change due to a customer's change in preference
will be charged at 50%of the original invoice prim for up to 60 days, and at
full invoice prim tbereafter. A wryest for a more expansive replacement will
require a charge for the difference in price.
Non -Adapt for Progressive Addition l<nses
If the end -user is unable to adapt to their progressive addition lenses,
AOSafsty will gladly replace the lemea with any mulblecal lees at no
charge within 60 days limn date of purchase. After 60 days, replacement
lenses will be charged at full prim.
Returns
Fabrication emirs, dotor's prescription changes, optician's changes, and
customer changes should be telephoned to an AOSafety Customer Service
Center so that the appropriate action can be taken.

Proposed Products and Services:
AOSafety specializes in protective eyewear. Our optical manufacturing facilities, testing
criteria, adherence to ANSI Z.87 standards, and third party certification all illustrate our
commitment to providing the finest prescription safety eyewear available.
We have developed our own line of industrial safety frames that are unique to the industry.
All commercially available frame models seem to be universally offered by every
competitor, we carry those frames as well. By offering about 50 additional styles of our
own, we can honestly claim to have the most complete selection available.
We sit on the ANSI board. Through our efforts we have help establish parameters for
safety eyewear production that have been adopted industry -wide. The differences between
dress and safety glasses are significant. Working with a company that truly understands
those differences ensures safety glasses that meet all legal requirements without
compromise.
The package price plan that the City of Fort Collins has used up to this point includes a
great many options at one price. We will continue to offer that strategy, or we can provide
an "a -la -carte" methodology if desired.
From a "bottom line" perspective all glasses will average out about the same cost -
wise using a weighted average. When comparing individual line items to a package
price scenario, you need to analyze multiple jobs. In preparing this proposal, we
evaluated a one year usage for all lens types, frames, tints, and options to gauge
our proposed price. Based upon our analysis, we can continue to offer the same
price for another contract term with no increases or price adjustments.
Dispensing:
Dispensing will be handled off -site.
I have enclosed a detailed list of current Eye Care Providers in the Fort Collins area.
Please visit out web site, www.aosafetysrx.com and click on the "Professional Eye Care
Locator" tool. Should additional off -site dispensing locations be required, AOSafety will
coordinate efforts to add such dispensing resources and provide a fitting kit of frame
samples to the ECP at no charge to the City of Fort Collins.
In this dispensing method, the opticians are capable of accurately taking the required
optical measurements, assisting with frame and lens selection, and fitting and delivering
the finished eyewear to your employees. The current negotiated rate is $ 20.00 per pair.
This cost is billed as a separate line item on the invoice.
Pricing:
The attached price list illustrates our material quote for the City of Fort Collins. Prices are

quoted firm and fixed for a two-year period with an option for two (2) - one-year renewals
based on price renegotiation. Freight is included in all pricing, payment terms, Internet
billing and related administrative details are as specified in the request for quotation.

All program details such as products allowed and not allowed and co -pay options (if
applicable) are represented on each form. This eliminates the guesswork typical
with safety prescription eyewear programs and helps reduce the possibility of
employees obtaining items not approved. Our Customer Service team verges each
order during the order entry process to ensure accurate invoicing and order
processing. The profile order forms are location specific.
2. Enhanced Frame Availability:
Designed with end -user acceptance in mind, AOSafety frame styles feature styles
and designs not found elsewhere. Please see our web site and the enclosed
product guide for more information.
3. Management Capabilities:
Aearo Company has invested in a nationwide team of management professionals.
In the Colorado area we have Stephanie Hoff -Walters in that role. Stephanie is
empowered to decide service concerns and address employee questions that may
arise from time to time. This provides a direct resource for city employees that is
unique in this industry.
4. Eye Care Professional Autopaye:
With other vendors, paying the ECP's can be an issue. AOSafety's exclusive
AutoPay system automatically pays the ECP for their dispensing services, ensuring
that dispensing services are uninterrupted.
5. AOSRx Solutionsw
The typical problems associated with prescription safety eyewear programs are
greatly minimized by using AOSRx Solutionse. On-line rules, a customized frame
catalog, eligibility management and a custom ECP panel are some of the available
tools that are provided at no cost. We would be delighted to demonstrate this

6. Invoicing Options:
In addition to traditional invoicing, we now offer customized EXCEL billing options.
This permits detailed information to be provided in a sort -able format that can help
with accounting and budgeting issues.

provided as a desk -top hyperlink for your prescription eyewear coordinators. This
link accesses a live database that populates an order form with current company
