1. Reason for Change: Additional Design Services Required by increasing the size of the Salt
Dome and deleting the loading ramp and adding a loading pit.
2. Description & Cost of Change: See Attached Vaught -Frye letter Authorization for Additional
Services, dated January 3, 2005.
3. Change in Contract Cost: Nineteen Thousand, Five Hundred, Sixty Eight Dollars ($19,568.00)
4 Change in contract time. Extended to October 31, 2005.
