Dcar Carol,
As promised, I am hereby providing you with pricing for the additional repair items
that tine looked at with Ray last Friday (August 131h) at the Old Town Parking
Structure. The cost of the additional items of work being priced includes all
labor/burden,, equipment, supplies, safety allowances (barricading), and permancltt
materials but excludes sales tax. I will list the hellos separately and will offer a
deductive price for the work that was under our original scope of work but deleted
due to the southwest stair tower being redone recently before any or Blazer's work
was started.
Item I- Removal and Replacement of the Existing Sealants and Pre -molded
Txpatasion Joint Assembly at the Pedestrian Rridge Connections from Garage
to Northwest Elevator Tower on Levels 213 & 4.
Blazer's scope of work for this item will be the complete removal mid replacement in
kind with all new materials of the existing sealants and pre -molded expansion joint
assemblies at the pedestrian bridge connectiorss (at the structure and at the olevator
connections) from the parking garage to the elevator tower at levels 2, 3 &s 4, 0 aid
price for this item of work is S2,070,00 Lump Suns or *boat $690.00 per level. CC

Isom 2; Seal Both (sets (inside/outsids) and Both Elevations (north/south) of %
Now SW Stair Canopy with Sealant Cant and Install Deck Coating out from the
CAnapy for a Distance of Three Feet.
Blazers scope of work for this Item will be to install a sealant cant at the underside
or new steel walls of the notch and south elevations of the new canopy that currently
has left a gap ranging from IN' to almost 1 K" under the steel walls. Once the gap is
scaled, a traffic coating will be applied from the sealant cants to a distance of no less
than three feet from the new stair enclosure. The concrete wall elevation (east side)
will receive the same treatment as described for the north sad south elevations,
Dlaaer will also make the 3-4 each overhead repairs to the spailed concrete that were
cmiscd when the new stairs were installed. Our price for this particular item of work
is S720.00 Lamp Sum.

Item 3: Install New Sealant cants at the Stair Treads/Risers and Landings of
New Southwest Stair Tower.
Blazer's scope of work for this item will be to clean the concrete: stair treads, risers
and landings then installing a contirmous sealant cant along the stringer to tread/riser
and )arming perimeters, using a low modulus i part urethane -based sealant such as
SikaFlex 15a. Our price for this item of work is $420.00 Lump Sum.
Item 4: Apply Penetrating Sealer to New Concrete of Stair Treads and
Landings of Now Southwest Stair Tower.
Blazer's scope of work for this item will be to clean the concrete stair treads and
landing of the new stair tower, after the sealant cant lw been installed (see Item 03)
and apply s 20% solids silane peAetrating sealer. Our price for this item of work is
$380.00 Lump Sum.
Item g: Remove and Replace Failing Concrete Cricket (approximately 150 SY)
on the Lower portion of the Fait Dpper Deck just South of the Ramp.
Alazer's scope of work for this item will be similar to the remedial patch work
(infill) that will be done on the larger area on the upper portion of the top deck befog
the deck coating work, BIazer will remove and legally dispose of the existing failed
concrete cricket and place a new sloped cricket to promote drainage away from this
corner of the parking structure. Once the cricket is installed and allowed to care, it
will be deck coated to help preserve the integrity of the new sloped patch. Our price
for this item of work is $3,918.00 Lump Sum,
Item G: Repair Failed Concrete at the Level 1 Transition from the Parking
Structure to the Concrete Apron at the Remington Entrance/Elit.
Blazer's scope of work for this item will be to remove the failed concrete
(approximately 4-5 inches in width) by depth of concrete slab and replace with new
elastomeric concrete end darn material such as Watson Bowman's WaboCrete,
respecting the control joint at this location, Our price for this item of work is
$1,450.00 Lump Sam.
Item 7., Rout and Seal Random Cracks (approximately 1300 lineal feet) and
Repair Four Each (@I SF leach) Spalled Area or Slab -on -Grade (SOO)
Concrete on Level One.
Blazer's scope of work for this item will be to rout approximately 1500 lineal feet of
random cracks that were measured on the lower level of the parking garage and
patch four spalled areas measuring approximately I SF each. The routed cracks will
be cleaned and then Sealed with a multi -component polyurethane sealant. Our price
for this item of work is $4,420.00 Lump Suss.

Item 8: Doduct for Original Work Scheduled for the SW Stair Tower.
Since Blazer's original scope of work was affected when the SW stair tower was
rebuilt recently by others, wo offer a price deduction of ($2,365.00) for the undone
work.
