The battery replacentem figure includes the following: travel time, mileage, hatteries, disposal of old
bancries, normal hour insrallation by a J'l' Packard service technician. '1 his quote is valid for 30 days.
Sales tax, freight or in -side delivery is NOT included, unless stated on page 2. An estimate for
optional items and the following is available upon request: inside Delivery, Lift Gate Delivery,
Freight, Pamirs & Planning. However, each estimate is subject to additional hillings for costs in
excess of our estimate. Plcasc contact us concerning any obstruction, which may restrict access
and/or create additional installation time (i.e. stairways, narrow doorways, etc) Unless explicitly

you require the project to be completed off hours, please contact us so we can adjust our proposal.
As the batteries are shipped, you are assured of receiving batteries manufactured with the latest (late
possible for your delivery. Pricing for freight will be based on actual costs incurred and may vary
based on discounts or surcharges imposed by scivicing freight forwarders. Your freight charges will
hich0c costs incurred to ship the batteries to your site as well as inside delivery to the location for
the Ups/battery racks/battery cabincr. hwpedited delivery may be possible so please provide an
expected delivery date and shipping location when placing your order.
)"I' Packard will coordinate with you the anticipated arrival date of your batteries and the dare of the
bat rely replacentcmr. J'1' Packard requires a 7-10 day notification for schedubn, of all preventative
maintenance or battery replacement services. Based on battery manufacturer used, lead time is 2-6
weeks from receipt of signcd order.
You will receive a receil)t for the battery disposal showing the F.P.A. Certification number from the
disposal facility within 2 weeks of The battery replacement.
Prices for batteries and equipment are 1 C.?13 factory. Full payment is due upon completion of battery
tcplaccment work.

The undersigned hereby certifies that she is Joyce Grenz, Accounts Payable
Representative of City of Fort Collins to execute this certificate and that the article or
articles specified in the accompanying orders, or on the reverse side hereof, are purchased
from JT PACKARD & ASSOCIATES. INC. for
the exclusive use of the City of Fort Collins of the State of Colorado.
It is understood that the exemption from tax in the case of sales of articles under this
exemption certificate to a State, etc., is limited to the sale of articles purchased for its
exclusive use, and it is agreed that if articles purchased tax free under this exemption
certificate are used otherwise or are sold to employees or others, such fact will be
reported by me to the manufacturer of the article or articles covered by this certificate. It
is also understood that the fraudulent use of this certificate to secure exemption will
subject the undersigned and all guilty parties to a fine of not more than $10,000.00 or to
imprisonment for not more than five years, or both, together with costs of prosecution.
