Commencement Date: 03/03/2004 Completion Date: 03/31/2004 Maximum Fee: (fixed fee): $3,300.00 Project Description: Complete Phase I ESA to comply with ASTM standard designation E1527- 00 at the Proposed Police Station Site (Parcel Numbers 87292-00-907 and 87292-00-915) located on South Timberline Road in Fort Collins, Colorado. Scope of Services: Phase I ESA including historic research, interview with owners, site inspection, and provide three copies of final report. Final report will be delivered on or before 03/3112004. Acceptance: ser Professional agrees to perform the services identified above and on the attached forms in accordance with the terms and conditions contained herein and in the Professional Services Agreement between the parties. In the event of a conflict between or ambiguity in the terms of the Professional Services Agreement and this work order (including the attached forms) the Professional Services Agreement shall control. Database and Records Review - The records review will provide information regarding activities which may have had an adverse impact on the property, and includes searching the following records and search distances: Federal and State Records (per ASTMstandard requirements specified in designation ASTM E1527-00) Record Searched Radius Searched CERCLIS Sites %: mile NPL Sites 1 mile Leaking Underground Storage Tanks (LUSTS) % mile Colorado Landfills Lists Y2 rile Colorado Registered USTs/ASTs adjacent sites RCRA TSD Facilities 1 mile RCRA Generators adjacent sites RCRA Corrective Action Sites 1 mile ERNS List site only State Superfund Sites 1 mile The records review will also include a review of the following agency records • available city or county planning, zoning, and building records, historic Sanborn maps, and historic topographic maps to identify previous land uses; • local, state, and federal regulatory agencies' records for public information related to possible hazardous material releases on the subject and surrounding properties; • available historical aerial photographs; • readily available data pertaining to site geologic, hydrologic, and subsurface conditions; interviews with available local and state agencies to help determine if any hazardous materials incidents or releases have occurred in the area that may involve the subject property_ c:\windowitdesktop'avalsh dxnments`ndminuttuive\mvktting\phac i ytopoml - scope orvo6 -police nationdx.07.02.00 Mar 02 04 04:04p WRLSH EMU 970 223 8577 p.7 �`� a&sll Environmental Scientists and Engineers, LLC SCOPE OF SERVICES (continued) Phase I Environmental Site Assessment Proposed Police Station Property, (Parcels 87292-00-907 and 87292-00-915), South Timberline Road, Fort Collins, Colorado Task 2. Site Inspection - A physical inspection of the subject property and, if necessary, adjacent properties will be conducted to complement the findings of the records review, to identify past and present land use practices, and to identify potential environmental liabilities. This task will: • identify visual indications of environmental contamination, land use practices, waste materials, suspect building materials, and other potential liabilities on the subject site and adjacent properties; • conduct interview(s) with current site owners/occupants and complete ASTM questionnaire; • examine any transformers located on site for conspicuous leaks involving potential PCB containing fluids; • obtain photographic documentation of the current site conditions and pertinent site features. Task 3. Report - The final reports will compile the findings of the investigation along with copies of relevant documents as exhibits. The report will at a minimum: • present factual information and WALSH's professional opinion regarding the presence or absence of contamination at the subject properties; • provide documentation of all Phase I Environmental Site Assessment activities; • include a statement of assumptions used in conducting the assessment and a review of its limitations; • present recommendations for further investigations, sampling, or corrective actions, if any. c\windoses'dcskwp%.�dsh i propoml - scope of.•mk - policc mubn doc.07.020A 2
