HomeMy WebLinkAbout244325 TURNING POINT FOR YOUTH & FAMILY DEVELOPMEN - CONTRACT - AGREEMENT MISC - TURNING POINTAMENDMENT TO THE RECIPIENT CONTRACT FOR COMMUNITY DEVELOPMENT BLOCK GRANT FUNDING THIS AGREEMENT is made and entered into this Ir day of 2004, by and between THE CITY OF FORT COLLINS, CO RAD , a Colorado municipal corporation (hereinafter referred to as "the City'), and TURNING POINT CENTER FOR YOUTH AND FAMILY DEVELOPMENT, INC., a/k/a Turning Point (hereinafter referred to as "the Subrecipient"), WITNESSETH: WHEREAS, the parties hereto previously entered into a Recipient Contract for Community Development Block Grant (CDBG) Funding executed November 5, 2002 (the "Contract") which provided for the rendition of certain services by the Subrecipient and a distribution by the City to the Subrecipient of certain funds (ONE HUNDRED THOUSAND DOLLARS - $100,000) made available to the City from the United States Government under Title I of the Housing and Community Development Act of 1974; and WHEREAS, the parties hereto are desirous of modifying the contract with respect to the scope of eligible activities thereunder, and WHEREAS, the CDBG Commission, on January 8, 2004, did by unanimous vote approve the amendments as attached (Exhibit A - Amended) to the Subrecipient's Scope of Services. NOW, THEREFORE, in consideration of the mutual promises of the parties and other good and valuable consideration, the receipt and adequacy of which is hereby acknowledged, the parties agree as follows: 1. See Attached: Exhibit A - Amended, Scope of Services. 2. All terns and conditions remain in effect except as modified by this amendment. EXHIBIT A - AMENDED SCOPE OF SERVICES 1. Project Description and Objectives. Turning Point Center for Youth and Family Development will purchase the six -unit property located at 207 E. Stuart St., Ft. Collins, CO. The property consists of four 1-bedroom units and two 2- bedroom units, and will provide transitional housing for up to ten youths/young adults, ages 15-26. Five ofthe six housing units will provide affordable Housing for income -qualified: clients. Aft e- On -site staff members will manage the facility and connect the residents to services as needed. Income levels of the residents will range from 0% to 50% of Area Median Income (AMI). The units will continue to be affordable to very low and low-income residents for a period of twenty years. The residents will be working towards independence by completing education, obtaining jobs, and receiving counseling and other services. Community Development Block Grant (CDBG) funds will be used for acquisition of the real estate described in Exhibit C. The balance of the funds necessary to purchase the property will be raised by the Subrecipient. IN WITNESS WHEREOF, the parties have executed this agreement the day and year first above written.
