Re: Life and LTD Plans
Dear Ms. Schafer:
The City of Fort Collins wishes to extend the agreement term for the above captioned proposal.
The term will be extended for one (1) additional year, January 1, 2001 through December 31,
2001.
If renewal at current rates is not acceptable for all life and LTD plan premium rates, effective
January 1, 2001, and you are requesting a change in premium rates, you must provide the
calculations that are used to develop these new rates. Please also send copies to Vincent
Pascale, at the City of Fort Collins Human Resources and Phil Goldstein of William M. Mercer,
Inc. '
If the renewal at current rates is acceptable to your firm, please sign this letter in the space
provided and return it to the City of Fort Collins, Purchasing Division, P. O. Box 580, Fort
Collins, CO 85022.
Please respond in the next 15 days.

LTD plans with Sun Life
We are currently working on the renewal pricing with Vincent Pascale and Phil
Goldstein. Once we have completed the renewal I will communicate it to Vincent and
Phil.

Subject: City of Fort Collins - January 1, 2001 Renewal
Dear Beth:
As you are aware, the City of Fort Collins' (the City) life and long-term disability plans
renew effective January 1, 2001.
Please provide us with the renewal for all life and LTD plan premium rates, effective
January 1, 2001. In addition to receiving the new premium rates, we would like to see the
calculations that are used to develop these new rates.
We need to receive this renewal as soon as possible. Please also send copies of the
renewal to Vincent Pascale at the City, and Phil Goldstein of William M. Mercer, Inc. at
