The term will be extended for one (1) additional year, January 1, 2003 through December 31, 2003. If the renewal is acceptable to your firm, please sign this letter in the space provided and return it to the City of Fort Collins, Purchasing Division, P. O. Box 580, Fort Collins, CO 85022, within the next fifteen days. If this extension is not agreeable with your firm, we ask that you send us a written notice stating that you do not wish to renew the contract and state the reason for non -renewal. Box 580 °- Fort Collins, CO 80522-0580 RE: Anthem Life Policy #006518 Dear Ms. Anderson: City of Fort Collins will be renewing their Voluntary Group Life and Voluntary Accidental Death and Dismemberment coverages with Anthem Life Insurance Company on January 01, 2003. The age rates for your Voluntary Group Life and Voluntary Accidental Death and Dismemberment programs have been reviewed based on the demographics, including age, gender and size of your group. We are pleased to be able to reduce your current age rates for your Voluntary Group Life as well as your Voluntary Accidental Death and Dismemberment coverages until the next anniversary of your plan. (Please contact our office if the actual number of eligible employees is different than the figure indicated in this letter.) Anthem Life's requirement states the greater of five (5) employees or 15% of the eligible employees must be enrolled in the Voluntary Group Life and Voluntary Accidental Death and Dismemberment programs. Acceptance of new enrollees may be subject to evidence of insurability. You will be receiving notification of the new premium Case baf:nc Anthem Life PRODUCT OVERVIEW GROUP TERM LIFE AND ACCIDENTAL DEATH & DISMEMBERMENT INSURANCE Group Life Insurance plans are available with a maximum benefit of $50,000 for groups of one to nine eligible employees, $300.000 for groups of 10-34 eligible employees and $500,000 for groups of 35 or more eligible employees. The minimum benefit amount available is $10,000 per employee ($20,000 minimum if sold with an Anthem Blue Cross and Blue Shield Health plan). Accidental Death & Dismemberment Insurance will provide an additional benefit equal to the Group Term Life coverage in the event of an employee's accidental death. In the event of an accidental loss of limb or sight, a percentage of the Accidental Death benefit will be paid. A Seat Belt Rider, which is also included on composite rated groups of 10 or more lives, pays an additional benefit if the insured suffers loss of life or limb in an automobile accident and there is specific evidence to show that a seat belt was worn. The Living Benefits Rider, which is a standard feature of the program, allows terminally ill employees to receive up to 50% (to a maximum of $100,000) of their life insurance benefit in a lump sum prior to death. SHORT TERM DISABILITY INSURANCE This product provides benefits, once the elimination period is satisfied, to partially replace lost income of employees who are disabled as a result of a non -occupational accident or sickness. Short Term Disability plans are available to groups as small as one life. Evidence of insurability is required for groups of less than 10 lives. Groups with 10 or more lives will receive guarantee issue. Benefits are payable if, as a result of a non -occupational accident or sickness, an employee is unable to perform any and every duty of his or her regular occupation. 3enefits can be a flat amount per week or a percentage of weekly income to a maximum of $500 - $1,000 per week depending on group size. Elimination periods are typically 1, 8, 15, or 31 days with benefit periods of 13, 26, or 52 weeks. LONG TERM DISABILITY INSURANCE This product provides monthly benefits to partially replace income lost as a result of any prolonged disability. Long Term Disability plans are available to groups as small as 3 lives. Evidence of insurability is required for groups of less than 10 lives. Groups with 10 or more lives will receive guarantee issue. For benefits to be payable, the employee must have a significant change in his or her physical or mental condition due to sickness or injury. The employee must also be prevented from performing, during the Benefit Qualifying Period and the following 24 months, the Essential Functions of his or her Regular Occupation or of a Reasonable Employment Option offered by the employer. As a result of the disability, the employee must be unable to earn more than 60% of his or her Pre -disability monthly income. After that time period, the employee must be prevented from performing the Essential Functions of any Gainful Occupation in which his or her training, education, and experience would allow him or her to perform and remain unable to earn 60% of his or her Pre -disability monthly income. Benefits are a percentage of monthly income with a typical maximum monthly benefit of $5,000. The maximum monthly benefit depends on the size of the group and salaries of highly compensated employees. The most common elimination period (EP) is 180 days with benefits payable to age 65 when disabled prior to age 63. Other EP and benefit duration options are available. Employee pjid Voluntary Group Term Life allows eligible employees and their dependents to obtain life insurance suited to their needs at no premium cost to your organization. This product is available to groups with as few as 5 lives and the greater of 5 lives or 25% of the eligible lives must participate in the program. Employees can apply for coverage from $10,000 to $300,000 or $500,000 depending on the size of the group. Spouses, under age 70, can apply for coverage up to the approved employee coverage amount as long as the spouse is not in active military service. 'f the employee does not enroll, spouse coverage is available to a maximum of $100,000 with Evidence of Insurability (no Guaranteed Issue). .Jnmarried dependent children from age 15 days to age 23 (age 25 if a student), who are not in active military service, are also eligible as long as the employee or spouse is enrolled for coverage. Coverage is portable and convertible and includes a Living Benefits Rider, which allows terminally ill employees to receive up to 50% (to a maximum of $100,000) of their life insurance benefit in a lump sum prior to death. VOLUNTARY ACCIDENTAL DEATH & DISMEMBERMENT INSURANCE This product, available to groups of 25 or more employees, provides 24-hour protection, on or off the job, in the event of accidental death or dismemberment at no premium cost to your organization. An employee can purchase either the Employee Only Plan or the Family Plan. Under the Family Plan, spouses under age 70 and all dependent children from birth to age 19 (age 25 if student) are covered. Voluntary Accidental Death & Dismemberment also includes five important benefit riders and a valuable Travel Assistance Program. VOLUNTARY SHORT TERM DISABILITY INSURANCE. Voluntary Short Term Disability provides benefits, once the elimination period is satisfied, to partially replace lost income of employees who are disabled as a result of a non -occupational accident or sickness at no premium cost to your organization. This product is available to groups with as few as 5 lives and the greater of 5 lives or 15% of the eligible lives must participate in the program. (Guaranteed Issue is available when 30% of the eligible lives participate and at lease 25 employees are enrolled.) Benefits are payable if, as a result of a non -occupational accident or sickness, an employee is unable to perform any and every duty of his or her regular occupation. Typical benefit design options: (other options are available) 1. A flat benefit of $300 per week not to exceed 70% of the employee's basic weekly income. 2. 60% of the employee's basic weekly income not to exceed $400 per week. Elimination periods can be 15, or 31 days and benefits can be paid for 13 or 26 weeks depending on the benefit option selected. VOLUNTARY LONG TERM DISABILITY INSURANCE Voluntary Long Term Disability Insurance provides monthly benefits to partially replace income lost as a result of any prolonged disability at no cost to your organization. This product is available to groups of at least 15 eligible lives and the greater of 15 lives or 25 % of the eligible lives must enroll in the program. The employee must have a significant change in his or her physical or mental condition due to sickness or injury.
