Scheduled maintenance services as listed in the specifications and conditions shall be billed and
reimbursed as follows:
1. Cost per inspection and service for three (3) Dover elevators:
• City Hall (300 Laporte Ave)
• Library (201 Peterson St.)
• Museum (200 Mathews St.) $ 240.00 per service
2. Cost per inspection and service for two (2) Westinghouse elevators:
• Police Services (300 Laporte Ave)
• Downtown Parking Garage (102 Remington) $ 160.00 per service
3. Cost per inspection and service for one (1) Montgomery elevator:
• Edora Pool/Ice Center (1801 Riverside Dr) $ 80.00 per service
4. Cost per inspection and service for eight (8) Schindler elevators:
• City Park Center (1599 City Park Ave)
• Building & Engineering (281 N. College Ave.)
• Civic Center Parking Structure (120 N Mason, 2-elevators)
• F C Senior Center (1200 Raintree Dr.)
• City Office Building (215 N Mason, 2-elevators)
• Mulberry Pool (424 W. Mulberry) $ 640.00 per service
Breakdown and repair, by the Contractor, of Customers' equipment shall be billed for time spent on
repairs exclusive of travel time. Billing and reimbursement shall be as follows:
• Regular rate

• Overtime rate (5:00 PM through 8:00 AM)

Material Costs:
Maximum allowable material markup, up to $500.00 = 10%. Over 500.00 = 8%
Service Provider mark-up for material costing up to $500.00: 10 % Over $500.00: 8 %
Material invoices are to be included with billing statements.

Method of Award
Award will be based on the cost of services rendered as set forth in the following sample invoice.
Pricing must come from the Bid Schedule, page 6. The sample invoice contains a combination of
the following:
• One maintenance service cost for each different elevator manufacturer
• Four (4) hours of regular time for a mechanic
• Two (2) hours of overtime for a mechanic
• Materials cost for two different jobs, one under $500 and one over $500.

2. Mechanics' regular rate: $ 80.00 X 4 hours = $ 320.00

4. $165.00 materials X Service Provider mark-up 10 % _ $ 181.50
5. $600.00 materials X Service Provider mark-up _8 % _ $ 648.00

STEVE LIEN
Elevator industry starting date: June 1992
Steve began his career with KONE Inc. and worked as a helper for six years. He
has since become certified as an Elevator Technician with his own route and is
now our lead mechanic in the Fort Collins/Greeley area. Steve is well versed in
all make and types of vertical transportation equipment.

ROY ALMS
Elevator industry starting date: June 1978
Roy began his career with KONE Inc. in February of 1981. He has been an asset
to our organization since day one. He is well schooled on all types of elevator
equipment and continues to stay on top of his profession by attending various
schools offered by the branch and our corporate offices. He has worked in
construction, repair, and maintenance holding the position of helper and
graduating to the position of mechanic. He is a lead serviceman and an
excellent troubleshooter.
MIKE EYRES
Elevator industry starting date: January 1967
Mike began his tenure with KONE Inc. in March of 1974. He brought with him
approximately seven years of experience in various positions. He began his
career as a helper and quickly progressed through the ranks to his current
position as our Cheyenne, Wyoming area foreman. Through the years, Mike has
continued to stay on top of his profession by attending various schools offered
by the branch and our corporate offices. He is well schooled on all types of
elevator equipment and continues to stay on top of his profession.
