Project Description: Reroof of existing Water Quality Laboratory Main Roof
Scope of Services: Removal of existing roof ballast where applicable preparation of existing BUR
installation of insulation, and installation of a fully adhered GenFlex 45 mil TPO membrane in accordance
with Genflex Specifications. Remove and reinstall all coping material and drain rings Upon competion
provide owner with 15-year full systems warranty. (Also see attached scope)
Service Provider agrees to perform the services
identified above and on the attached forms in
accordance with the terms and conditions
contained herein and in the Services Agreement
between the parties. In the event of a conflict
between or ambiguity in the terms of the
Services Agreement and this work order
(including the attached forms) the Services
Agreement shall control.
The attached forms consisting of one (1) pages
are hereby accepted and incorporated herein, by
this reference, and Notice to Proceed is hereby
given.

— Furnish material and labor to install a GenFlex 45 mil thermoplastic polyolefin
membrane roofing system fully adhered to a single layer of GenFlex high -density
wood fiberboard glued to the existing B.U.R. membrane. Remove and reinstall
all coping metal and drain rings. This system meets U.L. Class A fire rating
requirements and carries a manufacturer's 15-year full systems warranty with no
dollar limit.
— The following pricing is in accordance with The City of Fort Collins City
Services Contract.
— Laboratory Roof
Not to exceed $24,900.00.
— Unit pricing for wet insulation replacement - $1.35 per square foot.
