Reason fog Change: Additional design services to complete the Master Planning and
preliminary design services for the Civil Engineering work.
2. Description of Change: See attached letter dated June 19, 2001, from Vaught -Frye Architects.
3. Seventeen Thousand Six Hundred dollars ($17,600.00)
4. No impact on time.

Dear Jack:
In order for us to complete the Master Planning and Preliminary Phase civil work, for the
above referenced project, at 700 Wood Street, we are requesting additional design fees be
approved for the following amount, based on an hourly not -to -exceed basis:
Vaught*Frye Architects:
1) Additional work to complete contract: sub -total $12,000.00
Sear -Brown Group:
1) See attached breakdown of fees: sub -total $5,600.00
Total fee $17,600.00
If this is acceptable, please issue a Change Order, to our original contract, for this work.
